血清Hcy、Cys C联合尿足细胞检测对2型糖尿病肾病的早期诊断价值

【摘要】:目的探讨血清同型半胱氨酸(Hcy)、胱抑素C(Cys C)联合尿足细胞检测对2型糖尿病肾病(DN)的早期诊断价值。方法收集130例临床诊断为2型糖尿病患者,依据尿白蛋白/肌酐比值(UACR)将其分为正常白蛋白尿(NA)组42例、微量白蛋白尿(MA)组45例和大量白蛋白尿(LA)组43例;选取同期健康体检者40例作为对照组。应用全自动生化分析仪检测血清Hcy和Cys C水平,采用流式细胞术检测尿中足细胞数量;分析血清Hcy、Cys C、尿足细胞与UACR的关系,并应用ROC曲线确定单一和联合检测诊断早期DN的价值。结果 NA、MA、LA组血清Hcy、Cys C水平及尿足细胞数量均高于对照组,并随着DN进展而升高(P均0.05)。相关分析结果表明,血清Hcy、Cys C水平及尿足细胞数量分别与UACR呈正相关(r=0.83、0.83、0.84,P均0.05)。在NA组中,Hcy、Cys C和足细胞诊断早期DN的敏感性分别为59.5%、80.9%和64.3%,三者联合检测的敏感性为90.8%。结论 2型糖尿病患者肾损伤时血清Hcy、Cys C水平和尿足细胞数量显著升高,三者联合检测对于DN早期诊断具有重要价值。
[Abstract]:Objective to investigate the value of serum homocysteine cystatin, cystatin Cys Cys and urinary podocyte in the early diagnosis of type 2 diabetic nephropathy. Methods 130 patients with type 2 diabetes mellitus were collected. According to urinary albumin / creatinine ratio UACRs were divided into normal albuminuria (NAA) group (n = 42), microalbuminuria (MAA) group (n = 45) and large albuminuria group (n = 43). Serum Hcy and Cys C levels were measured by automatic biochemical analyzer, the number of podocytes in urine was detected by flow cytometry, and the relationship between urinary podocytes and UACR was analyzed. ROC curve was used to determine the value of single and combined detection in the diagnosis of early DN. Results the levels of serum Hcytys C and the number of urinary podocytes in NAA MAGLA group were higher than those in the control group, and increased with the progression of DN (P < 0.05). There was a positive correlation between the level of serum Hcytys C and the number of urinary podocytes and UACR. In na group, the sensitivity of HcyCys C and podocyte in diagnosing early DN were 59.5% and 64.3%, respectively. The sensitivity of the combined detection was 90.8%. Conclusion Type 2 diabetes mellitus has a sensitivity of 90.80.Conclusion Type 2 diabetes mellitus has a sensitivity of 90.8%. During renal injury, the levels of serum HcyCys C and the number of urinary podocytes were significantly increased. The combined detection has important value for early diagnosis of DN.
