终末期肾病患者血清淀粉样蛋白A的水平测定及意义

【摘要】:目的:本实验通过测定SAA和CRP、TCH、HDL的水平,研究ESRD患者体内SAA水平的变化情况,及SAA与CRP、TCH、HDL的相关性,探讨SAA在ESRD患者体内微炎症状态、脂质代谢紊乱中的作用。 方法:选取青海大学附属医院血液透析中心进行血液透析患者30例,肾病内科CKD-5期未进行透析的患者30例,选择体检者30例为对照组。分别收集三组各项指标值。所采集的数据使用SPSS17.0进行统计分析,计量资料采用x±S表示,不同组别间使用单因素方差分析,两两比较采用LSD分析,相关性分析采用Person分析,检验水准为ɑ=0.05。 结果:1、SAA、CRP和TCH水平在透析组最高,非透析组次之,对照组的表达水平最低;而HDL在对照组表达水平最高,非透析组次之,透析组最低。2、相关性分析提示,在对照组中SAA与CRP呈正相关,与TCH、HDL无相关性。在非透析组和透析组中,SAA均与CRP、TCH呈正相关,与HDL呈负相关。 结论:1、终末期肾病患者体内存在微炎症反应和脂质代谢紊乱;血液透析患者体内微炎症反应和脂质代谢紊乱较非透析患者重。2、终末期肾病,尤其是血液透析患者体内SAA表达水平升高,参与并加重了微炎症反应和脂质代谢紊乱,有可能进一步提高其心血管疾病发生率。
[Abstract]:Objective: to study the changes of SAA levels in patients with ESRD and the correlation between SAA and TCHH HDL, and to explore the role of SAA in microinflammation and lipid metabolism disorder in ESRD patients. Methods: thirty hemodialysis patients were selected from hemodialysis center of Qinghai University affiliated hospital, and 30 patients without dialysis in CKD-5 phase of nephropathy department. 30 cases of physical examination were selected as control group. Three groups of index values were collected. The collected data were statistically analyzed by SPSS17.0. The measurement data were expressed by x 卤S, single factor ANOVA was used among different groups, and LSD analysis was used to compare the data between two groups. Correlation analysis using Person analysis, the test level is 0. 05. Results the levels of HDL and TCH were the highest in dialysis group, followed by non-dialysis group, and the lowest in control group, while the expression level of HDL was the highest in control group, followed by non-dialysis group, and lowest in dialysis group. In the control group, there was a positive correlation between SAA and CRP, but no correlation with CRP, but a positive correlation between SAA and HDL in both non-dialysis group and dialysis group. Conclusion there are microinflammatory reaction and lipid metabolism disorder in patients with end-stage nephropathy, and microinflammatory reaction and lipid metabolism disorder in hemodialysis patients are more serious than those in non-dialysis patients. Especially in hemodialysis patients, the expression of SAA was increased, which involved in microinflammatory reaction and lipid metabolism disorder, which may further increase the incidence of cardiovascular disease.
