前列腺液Galectin-3浓度对前列腺癌诊断和治疗的临床价值

【摘要】：目的：研究前列腺液Galectin-3浓度对诊断前列腺癌，以及鉴别进展和转移型前列腺癌的临床应用价值方法：31名可疑为前列腺癌的中老年男性纳入为实验对象，均行经直肠B超引导下前列腺穿刺活检。在前列腺穿刺之前行经直肠前列腺按摩，取前列腺液。使用多克隆抗体ELISA法测前列腺液中Galectin-3浓度，使用人Galectin-3抗体免疫组化法对穿刺组织进行染色，比较前列腺癌组和前列腺增生组的Galectin-3浓度及组织表达的差异。结果：前列腺癌组和前列腺增生组相比，年龄与前列腺体积均无统计学差异。而PSA和PSAD有统计学差异，前列腺癌组大于前列腺增生组。前列腺液Galectin-3浓度前列腺癌组亦大于前列腺增生组（P0.001）。前列腺癌患者12人，其中有4人的骨核素扫描提示转移性骨损害，其Galectin-3浓度显著较其他前列腺癌者高。然而，Galectin-3和gleasson评分，PSA，，PSAD均无显著相关性。Galectin-3的ROC曲线下面积最大，为0.921，其诊断前列腺癌的价值大于PSA和PSAD.当Galectin-3浓度为4167ug/ml，敏感度为100%，特异度达到80%。免疫组化进一步显示，前列腺癌组织的Galectin-3表达量大于前列腺增生组，两组有统计学差异。结论：前列腺癌患者前列腺液的Galectin-3浓度显著高于良性前列腺增生者，并且，发生骨转移的前列腺癌Galectin-3浓度较未转移的患者更高。前列腺液Galectin-3不仅可以作为诊断前列腺癌的有利指标，也可以用来判断转移性前列腺癌的存在，从而指导治疗和推断预后。
[Abstract]:Objective: to study the clinical value of Galectin-3 concentration in prostatic fluid in the diagnosis of prostate cancer and the differential diagnosis and metastasis of prostate cancer.Methods 31 middle and aged men suspected to be prostate cancer were included in the study. All of them underwent transrectal ultrasound guided prostate biopsy.Before the prostate puncture, the prostate is massaged through the rectum and the prostatic fluid is taken.The concentration of Galectin-3 in prostatic fluid was measured by polyclonal antibody ELISA method, and the expression of Galectin-3 in prostate cancer group and prostatic hyperplasia group was compared by immunohistochemical staining with human Galectin-3 antibody.Results: there was no significant difference in age and prostate volume between prostate cancer group and prostatic hyperplasia group.But PSA and PSAD have statistical difference, prostate cancer group is bigger than prostatic hyperplasia group.The concentration of Galectin-3 in prostatic fluid in prostate cancer group was higher than that in prostatic hyperplasia group (P 0.001).Osteonuclide scanning of 4 of 12 patients with prostate cancer suggested metastatic bone damage, and their Galectin-3 levels were significantly higher than those of other prostate cancer patients.However, there was no significant correlation between Galectin-3 and gleasson score. The area under the ROC curve of Galectin-3 was the largest (0.921). The diagnostic value of Galectin-3 was higher than that of PSA and PSAD.When the concentration of Galectin-3 is 4167 ug.ml, the sensitivity is 100 and the specificity is 80.Immunohistochemical staining showed that the expression of Galectin-3 in prostate cancer was higher than that in benign prostatic hyperplasia (P < 0.05).Conclusion: the concentration of Galectin-3 in prostatic fluid of prostate cancer patients is significantly higher than that in patients with benign prostatic hyperplasia, and the concentration of Galectin-3 in prostate cancer with bone metastasis is higher than that in patients without metastasis.Prostatic fluid Galectin-3 can not only be used as a useful marker for the diagnosis of prostate cancer, but also can be used to judge the presence of metastatic prostate cancer, which can guide the treatment and predict the prognosis.
