雄性大鼠膀胱出口梗阻致膀胱重构的形态结构研究

【摘要】:目的 通过建立雄性大鼠膀胱出口部分梗阻模型,观察雄性大鼠膀胱出口部分梗阻后不同梗阻时期的膀胱逼尿肌细胞增生、凋亡及间质中胶原纤维的变化情况。 方法 选取Wistar雄性大鼠40只,通过前期对建立大鼠膀胱出口部分梗阻模型不同方法的比较,本实验研究采用经会阴途径球部尿道部分结扎的方法建立大鼠膀胱出口部分梗阻模型,随机分为假手术组10只,梗阻2周组15只,梗阻4周组15只。采用SABC免疫组化方法分析膀胱逼尿肌细胞增殖细胞核抗原PCNA表达情况;通过末端转移酶标记法(TUNEL法)检测膀胱逼尿肌细胞凋亡指数;运用VG染色法分析胶原纤维占膀胱逼尿肌面积比例变化情况。 结果 1、实验选取雄性Wistar大鼠40只,最终成活33只,,1只大鼠因麻醉过深死亡,2只大鼠因感染严重而死亡,4只大鼠因结扎过紧致膀胱破裂而死亡。 2、假手术组、梗阻2周组和梗阻4周组膀胱逼尿肌细胞PCNA阳性表达率分别为(17.19±1.37)%、(50.74±3.15)%、(33.58±2.80)%,经统计方差分析,梗阻组之间差异有统计学意义(P<0.05),梗阻组与假手术组之间差异有统计学意义(P<0.05)。 3、假手术组、梗阻2周组和梗阻4周组逼尿肌细胞凋亡指数分别为(20.22±1.70)%、(33.90±1.86)%、(51.18±3.30)%,经统计方差分析,梗阻组之间差异有统计学意义(P<0.05),梗阻组与假手术组之间差异有统计学意义(P<0.05)。 4、假手术组、梗阻2周组和梗阻4周组膀胱间质中胶原纤维占膀胱逼尿肌面积比例分别为(7.07±0.67)%、(13.19±1.50)%、(22.98±2.89)%,经统计方差分析,梗阻组之间差异有统计学意义(P<0.01),梗阻组与假手术组之间差异有统计学意义(P<0.01)。 结论 不同梗阻时期大鼠膀胱出口部分梗阻的膀胱逼尿肌形态结构的变化,可能是膀胱逼尿肌细胞增生、凋亡以及间质中胶原纤维占膀胱逼尿肌比例变化的综合作用结果。
[Abstract]:PurposeBy establishing a male rat model of partial bladder outlet obstruction, the proliferation, apoptosis and collagen fibers in interstitial tissue of bladder detrusor cells in different obstructive stages after partial bladder outlet obstruction in male rats were observed.MethodForty male Wistar rats were selected to establish the model of bladder outlet partial obstruction by comparing different methods in the early stage. The method of partial urethral ligation via perineal approach was used to establish the model of bladder outlet partial obstruction in rats.They were randomly divided into sham operation group (n = 10), obstruction group (n = 15) and obstruction group (n = 15).The expression of proliferating cell nuclear antigen (PCNA) in bladder detrusor cells was analyzed by SABC immunohistochemical method, and apoptosis index of bladder detrusor cells was detected by terminal transferase labeling method (Tunel).The ratio of collagen fibers to the area of bladder detrusor muscle was analyzed by VG staining.Result1. Forty male Wistar rats were selected and 33 rats survived. 2 rats died because of severe infection and 4 rats died of bladder rupture caused by too tight ligation.2. The positive expression rate of PCNA in bladder detrusor muscle cells in sham operation group, obstruction 2 week group and obstruction 4 week group was 17.19 卤1.37 and 50.74 卤3.15 respectively. The difference between obstruction group and sham operation group was statistically significant (P < 0.05), and there was significant difference between obstruction group and sham operation group (P < 0.05).3The apoptotic index of detrusor cells in sham operation group, obstruction 2 week group and obstruction 4 week group were 20.22 卤1.70 and 33.90 卤1.86 respectively. The difference was statistically significant (P < 0.05) between obstruction group and sham operation group (P < 0.05).(4) the proportion of collagen fibers in bladder stroma in sham operation group, obstruction 2 week group and obstruction 4 week group were 7.07 卤0.67 and 13.19 卤1.50 respectively.There was significant difference between obstruction group and sham operation group (P < 0.01).ConclusionThe morphologic changes of bladder detrusor in rats with partial bladder outlet obstruction at different stages of obstruction may be the result of the proliferation of bladder detrusor cells apoptosis and the changes of collagen fibers in bladder detrusor muscle.
