输尿管镜碎石术并发肾破裂治疗的临床研究

【摘要】：目的探讨输尿管镜碎石术并发肾脏破裂的可能原因及应对策略。方法收集该院行输尿管镜碎石术并发严重肾破裂的4例患者资料,分析其围术期临床资料,总结诊治经验。结果 4例患者出现患侧肾破裂大出血,3例为肾包膜下血肿,1例重度积液患者出血填塞集合系统,4例患者均通过保守治疗痊愈。结论输尿管镜碎石术并发肾脏破裂是一种少见但严重的并发症,同术前肾皮质的厚度和肾脏积液程度关系密切,另外泌尿系感染、术中冲水压力及熟练程度同肾脏损伤的发生也有一定的相关性。
[Abstract]:Objective to explore the possible causes and countermeasures of renal rupture after ureteroscopic lithotripsy. Methods the data of 4 cases of ureteroscopic lithotripsy complicated with severe renal rupture were collected, the clinical data during perioperative period were analyzed, and the experience of diagnosis and treatment was summarized. Results there were 4 cases of massive hemorrhage of the affected side of kidney rupture. 3 cases were subcapsular hematoma and 1 case was severe effusion. All the 4 cases were cured by conservative treatment. Conclusion Renal rupture complicated by ureteroscopic lithotripsy is a rare but serious complication, which is closely related to the thickness of renal cortex and the degree of renal effusion, and urinary tract infection. Intraoperative flush pressure and proficiency were also associated with renal injury.
