不同程度精索静脉曲张临床表现与性激素变化的相关性研究

【摘要】：目的:研究不同程度的精索静脉曲张(VC)临床表现与性激素变化的相关性及其特征,提出临床工作中对精索静脉曲张(VC)治疗的建议。方法:从2015年3月—2016年9月在门诊就诊的患者中,查体有精索静脉曲张表现、彩超证实有精索静脉形态学改变,根据两者可明确诊断为精索静脉曲张(VC),同时根据程度不同按照诊断标准将精索静曲张患者分为3个亚组(其中双侧精索静脉曲张时以较重侧为主),按照随机数字表法在各亚组中选取30例患者。我院泌尿科作为婚检定点医院,在同期体检的健康人群中按上述方法选取30例。对其外周血中卵泡刺激素(FSH)、黄体生成素(LH)和睾酮(T)水平测定,并进行比较观察。对各组患者进行临床表现问卷调查,根据VC患者临床表现分度表,对3组患者临床症状进行对比观察。结果:各病例组的血清FSH和LH水平均较对照组升高(P0.05),不同程度VC患者间FSH、LH间两两相比,得出F=7.38(P0.05),F=9.23(P0.05),结果差异有统计学意义,可以认为不同程度精索静脉曲张的FSH、LH水平不全相同;T水平在各组中较对照组均未见明显降低(P0.05),而每组之间进行比较,得出F=0.59(P0.05),结果无统计学意义,可以认为不同程度精索静脉曲张的T水平变化不大。在各病例组临床症状之间比较,得出?(17)(28)(18)(15)(13)(16)(20)(P0.05),结果差异有统计学意义,可以认为不同程度精索静脉曲张的临床表现不全相同,并且临床表现随着精索静脉曲张程度的加重而加重。结论:VC可引起患者外周血FSH、LH水平升高,说明VC对男性性激素确实有一定影响,并随不同程度精索静脉曲张而变化;VC患者的外周血T水平变化不显著,可能VC对睾丸的损害程度与临床程度无关;VC患者临床症状中表现来看,精索静脉曲张的临床表现随着精索静脉曲张程度的加重而加重,根据这一结论可以对精索静脉曲张治疗提出建议。
[Abstract]:Objective: to study the relationship between the clinical manifestations of varicocele and the changes of sex hormones, and to put forward some suggestions for the treatment of varicocele. Methods: from March 2015 to September 2016, patients with varicocele were examined, and morphological changes of spermatic veins were confirmed by color Doppler ultrasonography. The patients with varicocele can be definitely diagnosed as varicocele according to the degree of varicocele, and the patients with varicocele are divided into three subgroups according to the diagnostic criteria. The patients with varicocele in bilateral varicocele are divided into three subgroups. 30 patients in each subgroup were selected by tabular method. The urology department of our hospital, as a designated hospital for premarital examination, selected 30 cases according to the above method in the same period of physical examination of the healthy population. The levels of follicle stimulating hormone (FSH), luteinizing hormone (LH) and testosterone (T) in peripheral blood were measured and compared. The clinical symptoms of the three groups were compared and observed according to the clinical manifestation scale of VC patients. Results: the serum levels of FSH and LH in each case group were higher than those in the control group (P 0.05). The results showed that the serum levels of FSH and LH in patients with different degrees of VC were significantly higher than those in the control group. The results showed that the serum levels of FSH and LH in each group were significantly higher than those in the control group. It can be concluded that the levels of FSHL LH in varicocele of different degrees are not the same as those in the control group. The results showed that there was no significant difference in the level of FSH LH between each group and the control group, and the results were not statistically significant. The T level of varicocele with different degrees of varicocele may be considered to have little change. The clinical symptoms of each group were compared, and it was concluded that the clinical manifestations of varicocele of different degrees were different, and the clinical manifestations were aggravated with the exacerbation of varicocele. Conclusion the level of FSH LH in peripheral blood of patients with VC can be increased by 1: VC, which indicates that VC does have a certain effect on male sex hormone, and the changes of T level in peripheral blood of patients with VC with different degrees of varicocele are not significant. It is possible that the degree of testicular damage caused by VC is not related to the clinical degree. The clinical manifestation of varicocele is aggravated with the severity of varicocele. According to this conclusion, the treatment of varicocele can be recommended.
