【摘要】：目的：汇总分析我院逆行输尿管软镜激光碎石治疗肾结石并发症发生率,比较我院、国内及国外输尿管软镜治疗肾结石并发症的发生率的区别,分享我院降低并发症的措施及经验。方法：2009年10月至2013年10月邵逸夫医院逆行输尿管软镜钬激光碎石治疗肾结石病例500例,分析其并发症发生,并与国内、国外同类病例(1678例、747例)。比较出血、感染、输尿管损伤、石街形成等并发症发生率。并通过统计学分析我院在并发症防治方面的优劣势。结果：我院在输尿管镜钬激光治疗肾结石中,术中出血、输尿管损伤、术后肾周血肿、全身炎症反应综合征、感染性休克、输尿管穿孔发生率分别为：1.6%、1.4%、0.8%、2.6%、0.6%、0.4%。我院在出血、石街形成等方面与国内外并发症发生无明显差异。(vs国外：矫正卡方=0.00348,vs国内矫正卡方=3.8265,卡方=3.183,p值均0.05;vs国外：卡方=1.4539,P0.05)输尿管损伤较国内发病率低(矫正卡方=12.62,p0.05),感染性发热较国外发病率低(卡方=8.6714,p0.05)。结论：虽然输尿管软镜是微创手术,但是仍有可能发生各种并发症,应引起重视。
[Abstract]:Objective: to analyze the incidence of complications of retrograde soft ureteroscopy laser lithotripsy in the treatment of renal calculi, and to compare the incidence of complications between our hospital, domestic and foreign ureteral soft endoscopy. To share the measures and experiences of reducing complications in our hospital. Methods: from October 2009 to October 2013, 500 cases of renal calculi were treated with retrograde ureteroscopic holmium: YAG laser lithotripsy. Complications such as bleeding, infection, ureteral injury and stone street formation were compared. And through the statistical analysis of our hospital in the prevention and treatment of complications advantages and disadvantages. Results: the incidence of intraoperative bleeding, ureteral injury, perirenal hematoma, systemic inflammatory response syndrome, septic shock, and ureteral perforation in our hospital were 1. 6% and 1. 4% respectively. Our hospital is bleeding, There was no significant difference between the formation of stone street and complications at home and abroad. The incidence of ureteral injury was lower than that in China (correction of chi-square 12.62p0.05a, P = 0.05p = 3.183P = 0.05N). The incidence of ureteral injury in China was lower than that in China (correction of chi-square 12.62p0.05a), and the incidence of ureteral injury was lower than that of domestic patients (correction of chi-square 12.62p0.05a), and the incidence of ureteral injury was lower than that of domestic patients. The incidence of fever was lower than that of abroad. Conclusion: although soft ureteroscopy is a minimally invasive operation, complications may occur, which should be paid attention to.
