维持性透析患者体内氧化应激状态及影响因素研究

【摘要】:目的:探讨CKD5D期肾病患者体内氧化应激水平变化及相关影响因素,比较不同的肾替代治疗方式对终末期肾病患者体内氧化应激水平的影响。 方法:收集CKD5D期患者血清,采用双抗体夹心法测定血清超氧化物歧化酶(SOD)、丙二醛(MDA)和谷胱甘肽-过氧化物酶(GSH-Px)水平,同时收集患者临床资料,用统计学方法进行相关影响因素分析。 结果:CKD5D期患者氧化应激水平明显高于正常对照组氧化应激水平。在CKD5D期患者体内,维持性血液透析患者血清SOD水平低于维持性腹膜透析患者,MDA水平高于腹膜透析患者,均有统计学差异(p0.05);GSH-Px水平两组无统计学差异。多因素回归分析结果提示:CKD5D期患者体内SOD水平与是否服用血管紧张素转化酶抑制剂/血管紧张素II受体拮抗剂(ACEI/ARB)、透析龄、年龄和性别密切相关。进一步亚组分析发现:病因为高血压肾病的CKD5D期患者体内SOD水平与是否服用他汀类药物、年龄和性别有关,病因为糖尿病肾病的CKD5D期患者体内SOD水平与透析龄及年龄有关,病因为慢性肾小球肾炎的CKD5D期患者体内SOD水平与是否服用他汀类药物有关。 结论: CKD5D期患者体内氧化应激水平与透析龄、年龄及性别相关,贫血及炎症会加重氧化应激水平,维持性血液透析患者体内氧化应激水平高于维持性腹膜透析患者。
[Abstract]:Objective: To investigate the changes of oxidative stress and related factors in patients with CKD5D nephropathy, and to compare the effects of different renal replacement therapy on the level of oxidative stress in patients with end-stage renal disease.
Methods: the serum levels of CKD5D patients were collected and the levels of serum superoxide dismutase (SOD), malondialdehyde (MDA) and glutathione peroxidase (GSH-Px) were measured by double antibody sandwich method. The clinical data of patients were collected and the related factors were analyzed by statistical methods.
Results: the level of oxidative stress in patients with CKD5D was significantly higher than that of the normal control group. In the patients with CKD5D, the serum SOD level of the patients in the maintenance hemodialysis patients was lower than those in the maintenance peritoneal dialysis patients. The level of MDA was higher than that of the peritoneal dialysis patients (P0.05); there was no statistical difference between the two groups of GSH-Px. The regression analysis showed that SOD levels in patients with CKD5D were closely related to the age, age, and sex of the angiotensin converting enzyme inhibitor / angiotensin II receptor antagonist (ACEI/ARB). Further subgroup analysis found that the level of SOD in patients with CKD5D phase of hypertensive nephropathy and whether to take statins or not It is related to age and sex. The level of SOD in patients with phase CKD5D of diabetic nephropathy is related to the age and age of dialysis. The level of SOD in CKD5D patients with chronic glomerulonephritis is related to whether or not to take statins.
Conclusion: the levels of oxidative stress in CKD5D patients are related to age and age, age and sex. Anemia and inflammation may aggravate the level of oxidative stress. The level of oxidative stress in maintenance hemodialysis patients is higher than that of patients with maintenance peritoneal dialysis.
