绝经期女性泌尿道菌群特征及与尿路感染关系的研究

【摘要】:目的:通过16S rRNA基因高通量测序的非培养检测方法,研究伴有尿路感染症状的绝经期女性和健康绝经期女之间泌尿道细菌结构的差异。方法:收集就诊于青岛大学附属医院肾病内科门诊的有尿路感染症状的绝经期女性14例,留取其尿液,将尿液样本分为两份,一份进行细菌基因组DNA提取,应用16S rRNA高通量测序的方法检测尿液样本中细菌的种类;另外一份送至细菌室进行尿培养检查。收集同时期于青岛大学附属医院体检中心健康体检的绝经期女性,共14例,留取其尿液,同样进行细菌基因组DNA提取及16SrRNA测序。分析两组女性的泌尿道细菌结构之间差异。结果:(1)测序结果显示两组女性的尿液菌群构成均呈多样化,但个体差异较明显。在所有的尿液样本中,共检测出385个属的细菌类型,平均每个尿液样本包含25个属的细菌。其中检测到最丰富的细菌前四位分别是是厚壁菌门、放线菌门、变形菌门和拟杆菌门。在属的水平上,检测最多的前十位细菌为乳酸杆菌属、加德纳菌属、棒状杆菌属、肠杆菌科、葡萄球菌属、链球菌属、厌氧球菌属、嗜胨菌属、普氏菌属、放线菌属。(2)有尿路感染症状的绝经期女性尿液中加德纳菌属、气球菌属的检出率显著高于正常对照组,乳酸杆菌的检出率显著低于正常对照组,差异有统计学意义(P0.05)。其他细菌如肠杆菌科、厌氧球菌属、放线棒菌属、放线菌属、纤毛菌属、普氏菌属、解脲寡源杆菌属等在实验组中被检测到的比例较对照组高,在对照组中比例较高的菌属有链球菌属、产黄杆菌属、嗜胨菌属、双歧杆菌属等,但因本实验样本量少等原因,以上两组的差异不具有统计学意义(P0.05)。结论:绝经期女性的尿液菌群构成呈多样化,检测到最丰富的细菌是厚壁菌门、放线菌门、变形菌门和拟杆菌门。样本中以乳酸杆菌为优势菌的最多,其次是加德纳菌。有尿路感染症状的绝经期女性患者泌尿道菌群表现为乳酸杆菌显著减少,而加德纳菌属、气球菌数量增加。提示伴随尿路感染症状的绝经期女性泌尿道菌群结构发生了改变,可能是引起绝经期妇女尿路感染发病率升高的重要原因。
[Abstract]:Objective: to study the difference of urinary tract bacterial structure between menopausal women with urinary tract infection and healthy menopausal women by using 16s rRNA gene high-throughput sequencing method. Methods: fourteen postmenopausal women with urinary tract infection symptoms were collected from the Department of Nephrology, affiliated Hospital of Qingdao University. Urine samples were collected and divided into two parts. One sample was extracted from bacterial genome DNA. The bacterial species in urine samples were detected by 16s rRNA high-throughput sequencing, and the other samples were sent to the bacteria room for urine culture. A total of 14 postmenopausal women who were examined in the physical examination Center of affiliated Hospital of Qingdao University in the same period were collected. Urine samples were collected and the genomic DNA of bacteria was extracted and 16SrRNA sequenced. The differences of urinary tract bacterial structure between the two groups were analyzed. Results: the results of sequencing showed that the composition of urine flora of the two groups was diverse, but the individual difference was obvious. A total of 385 genera of bacteria were detected in all urine samples, with an average of 25 genera per urine sample. The four most abundant bacteria detected were phylum thunbergiformis, actinomycetes, Proteus and Bacteroides. At the genus level, the top 10 most detected bacteria were Lactobacillus, Gardnebacterium, Corynebacterium, Enterobacteriaceae, Staphylococcus, Streptococcus, Anaerobes, peptone, Platella, The detection rate of Gardnerella in menopausal women with urinary tract infection was significantly higher than that of normal controls, and the detection rate of Lactobacillus was significantly lower than that of normal controls (P 0.05). Other bacteria, such as Enterobacteriaceae, Anaerobes, Actinomycetes, Actinomyces, Cilium, Platinum, Urea-Oligomonas, and so on, were detected more frequently in the experimental group than in the control group. In the control group, there were streptococcus, xanthobacterium, peptone, bifidobacterium and so on. However, the difference between the two groups was not statistically significant because of the small sample size in this experiment. Conclusion: the composition of urine flora of menopausal women is diverse, and the most abundant bacteria detected are phylum thunbergiformis, actinomycetes, Proteus and Bacteroides. Lactobacillus was the dominant bacteria in the sample, followed by Gardner's. The urinary tract flora of menopausal women with urinary tract infection symptoms was significantly decreased, while the number of balloon bacteria increased in Gardner's genus. It is suggested that the changes of urinary tract flora in menopausal women with urinary tract infection symptoms may be an important reason for the increase of urinary tract infection in menopausal women.
