大连地区男性年龄相关前列腺特异性抗原参考值范围

【摘要】：目的分析大连地区男性血清前列腺特异性抗原(PSA)水平,探讨建立本地区男性年龄相关PSA参考值范围,以提高PSA对本地区早期前列腺癌诊断的准确性。方法 2011年1月至2016年6月,收集9 342例于大连医科大学附属第二医院健康体检男性的血液样本,排除存在下尿路症状者,最终共有8 453例成年男性纳入本研究。结果 8 453例健康男性PSA中位数为0.93μg/L,四分位数间距为0.81μg/L,95%百分位数为3.49μg/L。≤4.0μg/L的8 169例(96.6%);4.0~10.0μg/L的268例(3.2%);10.0μg/L的16例(0.2%);各个年龄组PSA 95%百分位数分别为:20~30岁0~1.67μg/L;30~40岁0~2.01μg/L;40~50岁0~2.41μg/L;50~60岁0~3.25μg/L;60~70岁0~3.92μg/L;70~80岁0~5.48μg/L;≥80岁0~9.35μg/L,各年龄组间均数差别有统计学意义(P0.01)。前列腺特异性抗原与年龄呈正相关(r=0.251,P0.001)。结论大连地区男性不同年龄段PSA水平存在明显差异,且均低于OESTERLING标准。筛查前列腺癌时,依据本地区年龄相关PSA参考值可能有益于前列腺癌的早期检出,降低过度诊疗的风险。
