超声诊断机器人辅助腹腔镜肾部分切除术后并发广泛腹壁皮下积气1例

【摘要】：正患者女,68岁。既往反复发作哮喘50余年,此次因体检腹部超声发现右肾肿物(大小约8.5cm×7.6cm),腹部CT检查诊断右肾血管平滑肌脂肪瘤。后于我院泌尿外科行全麻下机器人辅助腹腔镜肾部分切除术(右侧)。术前血分析、肝肾功能正常。手术顺利。术后6h患者哮喘发作,给予吸入糖皮质激素后症状控制。术后15h患者感右上腹胀痛,触诊右上腹壁存在"捻发音",轻微压痛,无反跳痛。行急诊床边消化系统超声检查:右侧腹壁皮下见细带状强回声(上至剑突下,下至脐下2cm,左至腹中线,右至腋后线),后伴宽大声影,致使肝右叶、部分肝
[Abstract]:The patient is 68 years old. For more than 50 years of recurrent asthma, the right renal mass (8.5cm 脳 7.6cm) was found by abdominal ultrasound, and the right renal angiomyolipoma was diagnosed by abdominal CT. Robo-assisted laparoscopic partial nephrectomy (right) under general anesthesia was performed in our urology department. Preoperative blood analysis showed normal liver and kidney function. The operation went well. Patients with asthma attack 6 hours after the operation were given symptoms control after inhaling glucocorticoid. 15 hours after operation, the patient felt distended pain in the right upper abdomen, and there was "twisted pronunciation" in the palpation of the right epigastric wall, with slight tenderness and no rebound pain. Emergency bedside digestive system ultrasound examination: the right abdominal wall subcutaneous thin band strong echo (up to the xiphoid process, down to the subumbilical 2 cm, left to the middle line, right to the posterior axillary line), followed by a broad sound shadow, resulting in the right lobe of the liver, part of the live
