冠脉介入术后尿NGAL、IL-18和血CysC在临床诊断CIN的价值

【摘要】:目的 研究中性粒细胞明胶酶相关脂质运载蛋白(Neutrophil Gelatinase-AssociatedLipocalin, NGAL)、白细胞介素-18(Interleukin-18, IL-18)、半胱氨酸蛋白酶抑制C(cystatinC, CysC)等肾脏损伤的生物学指标在冠状动脉造影(Coronary angiograph,CAG)介入(percutaneous coronary intervention, PCI)术后的变化及对对比剂肾病(Contrast Induced Nephropathy, CIN)早期诊断的临床预测价值。 方法 选取我院2012年5月~2014年1月期间180例行冠状动脉造影介入术治疗的患者为研究对象,,检测术前、术后1天、2天、3天血SCr、血CysC、尿IL-18和尿NGAL的水平。发生CIN的病例(以冠状动脉术后48h的血清肌酐(SCr)≥44.2mmol/L或升高≥25%为诊断标准)判为CIN组,未发生CIN的病例判为非CIN组。 结果 1180例行冠状动脉造影介入术治疗的患者中出现13例CIN患者,CIN的发生率为7.2%。 2术后第1天尿NGAL水平达到高峰,较术前基线水平显著升高(P0.05)。 3术后第2天尿IL-18水平达到高峰,同基础水平相比有显著差异(P0.05)。 4血CysC水平无明显性变化(P0.05)。 5尿NAGL, IL-18在ROC曲线下的面积分别为0.902、0.763,敏感性分别为79%、64%,特异性分别为58%、82%。 结论 1.尿NGAL是早期诊断急性肾损伤(AKI)的一种敏感的标志物。 2.尿IL-18在早期预测CIN中也具有一定的诊断价值。 3.尿NGAL较尿IL-18在预测CIN更加敏感,但NGAL单独诊断CIN受到许多潜在因素的干扰(如交叉反应物、内源性物质等)。
