少见病理亚型肾细胞癌11例动态增强CT表现及文献复习

【摘要】：目的总结几种少见病理亚型肾细胞癌的动态增强CT表现,提高影像诊断水平。方法结合相关文献,回顾性分析经手术病理证实的11例少见病理亚型肾癌患者动态增强CT表现。结果 5例乳头状细胞癌,4例平扫密度不均,1例为囊性低密度影,增强扫描呈少血供表现;4例嫌色细胞癌,3例平扫密度均匀,1例密度不均,增强扫描呈均匀或不均匀轻中度强化;1例Bellini集合管癌,平扫表现为肾脏体积增大,密度不均,肿瘤界限不清,增强扫描呈轻度不均匀延时强化,并侵犯肾周脂肪囊;1例Xp11.2易位/TFE3基因融合相关性肾癌,平扫病灶内见多发环形钙化灶,增强扫描病灶实性部分明显强化。结论 CT对少见病理亚型肾癌的定性诊断有一定的价值,但确诊还应依赖于病理学检查。
[Abstract]:Objective to summarize the dynamic enhanced CT findings of several rare pathological subtypes of renal cell carcinoma (RCC) and to improve the imaging diagnosis. Methods the dynamic enhanced CT findings of 11 patients with rare pathological subtypes of renal cell carcinoma confirmed by surgery and pathology were analyzed retrospectively. Results in 5 cases of papillary cell carcinoma, 4 cases of papillary cell carcinoma showed uneven density on plain scan, 1 case showed cystic low density, and 4 cases showed hypoglycemia on enhanced scan. 3 cases of chromophobe cell carcinoma showed homogeneous and even density in 1 case. One case of Bellini collecting duct carcinoma showed homogeneous or uneven enhancement on contrast enhanced scan. The plain scan showed that the renal volume was enlarged, the density was uneven, the tumor boundary was not clear, and the enhancement scan was slightly uneven and delayed enhancement. One case of Xp11.2 translocation / TFE3 gene fusion associated with renal cell carcinoma was involved in perirenal fat cysts. Multiple circular calcification foci were found in the lesions on plain scan. Conclusion CT is valuable for the qualitative diagnosis of rare renal cell carcinoma, but the diagnosis should depend on pathological examination.
