男性不育症患者勃起功能状况与精神心理状态的相关性研究

【摘要】：目的:通过分析男科特需门诊不育症患者的勃起功能障碍(ED)发生情况与精神心理状态,探讨不育症、勃起功能障碍与精神心理因素的相互关系。方法:选择男科特需门诊不育症患者252例,进行问卷调查,内容包括患者的一般情况、精液常规结果、性生活情况、国际勃起功能指数评分(IIEF-5)、患者抑郁自评工具(PHQ-9)和广泛性焦虑障碍量表(GAD-7)。调查不育症患者的勃起功能及抑郁、焦虑状况,统计其发生率,分析各症状间的相关性。以100例正常生育男性志愿者为对照组。结果:不育组245例、对照组97例完成调查。不育组患者中,"感觉疲惫或没有活力"和"变得很容易烦恼或急躁"分别成为抑郁和焦虑中最普遍的症状,其中,20.4%患者存在抑郁障碍,42.9%的患者存在焦虑障碍。不育组患者的PHQ-9、GAD-7评分明显高于对照组(P0.05);不育组患者的ED发生率为28.6%,明显高于对照组12.4%,且IIEF-5评分均较对照组明显降低(P0.01),其性生活频率、对性生活的满意度均较对照组下降(P0.05);不育症伴有ED患者,其PHQ-9、GAD-7评分明显高于不伴有ED的患者(P0.01)。Logistic回归分析显示:不育症伴ED组患者性欲的强弱、精液检查结果为抑郁障碍的危险因素;年龄、受教育程度、疾病病程及是否进行过辅助生殖为焦虑障碍的危险因素。结论:男科特需门诊不育症患者的心理健康水平较差,ED发生率较正常人群高,且与精神心理症状存在相互影响。
[Abstract]:Objective: to investigate the relationship between infertility, erectile dysfunction and psycho psychological factors by analyzing the incidence of erectile dysfunction (ED) and mental state of infertile patients in special outpatient department of andrology. Methods: 252 infertile patients with special needs of andrology were investigated by questionnaire, including the general condition of the patients, the results of semen routine, and the sexual life. International erectile function Index (IIEF-5), self-rating Depression scale (PHQ-9) and General anxiety Disorder (GAD-7). To investigate the erectile function, depression and anxiety in infertile patients, the incidence rate and the correlation between symptoms were analyzed. 100 normal male volunteers were taken as control group. Results: 245 cases of infertility group and 97 cases of control group completed the investigation. In infertile patients, "feeling tired or not active" and "becoming irritable or irritable" were the most common symptoms of depression and anxiety respectively. Among them, 20.4% of patients had depression disorder and 42.9% of them had anxiety disorder. The PHQ-9,GAD-7 score of the infertile patients was significantly higher than that of the control group (P0.05), the incidence of ED in the infertile patients was 28.6, which was significantly higher than that in the control group (12.4b), and the IIEF-5 score was significantly lower than that in the control group (P0.01). The PHQ-9,GAD-7 score of infertility with ED was significantly higher than that of patients without ED (P0.01). Logistic regression analysis showed that the sexual desire of infertility with ED group was stronger than that of the control group (P0.05), and the PHQ-9,GAD-7 score of infertility patients with ED was significantly higher than that of patients without ED (P0.01) .Logistic regression analysis showed that: the sexual desire of patients with infertility with ED was strong and weak. The results of semen examination were risk factors of depression, age, education level, course of disease and whether assisted reproduction were the risk factors of anxiety disorder. Conclusion: the incidence of Ed in infertile patients with special needs in andrology is higher than that in normal subjects, and there is an interaction between Ed and psychosomatic symptoms.
