Syn、CgA及Ki-67在前列腺癌中的表达及其临床意义

【摘要】:目的:探讨突触素(Synaptophysin, Syn)、嗜铬粒蛋白A(chrormogranin A, CgA)及Ki-67在前列腺癌组织中表达的临床意义及Ki-67与前列腺癌的神经内分泌化(neuroendocrine differentiation, NED)之间的关系,为前列腺癌在临床诊断及治疗提供有效依据。 方法:1.选择前列腺癌(PCa组)标本50例,其中高分化癌3例,中分化癌23例,低分化癌24例;因高分化前列腺例数较少,将高分化和中分化合为H组(Gleason积分≤7),低分化为L组(Gleason积分7),10例的良性前列腺增生标本(BPH组)。本实验以免疫组织化学染色法为实验方法,比较Syn、CgA、Ki-67在前列腺癌及良性前列腺增生中的表达及其差异,讨论Syn、CgA、Ki-67的阳性表达率与前列腺癌的分化、临床病理分级之间的关系。 结果:1.Syn阳性表达率分别为PCa组56%和BPH组10%,两组的比较有显著性差异(P0.05)。CgA阳性表达率PCa组48%和BPH组0%。两组比较有显著性差异(P0.05)。Ki-67阳性表达率分别为PCa组74%和BPH组40%。两组比较有显著性差异(P0.05)。 2.将前列腺癌组分成H组和L组。Syn、CgA、Ki-67的阳性率明显高于积分低的一组。Syn阳性表达率分别为:H组42.3%;L组70.8%。两组的比较有显著性差异(P0.05)。CgA阳性表达率分别为:H组34.6%;L组62.5%。两组的比较有显著性差异(P0.05)。Ki-67阳性表达率分别为:H组61.5%;L组87.5%。两组比较有显著性差异(P0.05)。 3.采用Syn及CgA同时阳性者为NED阳性,其余为NED阴性。H组神经内分泌化例数为9例。L组神经内分化阳性例数为15例。两组相比较有显著差异(P0.5)。 4.24例NED阳性者中Ki-67阳性例数为22例,其阳性率为91.7%,NED阴性者Ki-67阳性例数为13例,其阳性率为50%,两组的比较有显著性差异(P0.05)。 结论:1.Syn、CgA、Ki-67。在前列腺癌中阳性表达率明显高于良性前列腺增生。表明这三种指标可用于鉴别良性前列腺增生与前列腺癌。2.syn、CgA、Ki-67的阳性率随着前列腺癌的分化程度升高而降低,即低分化前列腺癌阳性率就升高。说明前列腺癌的发生和发展均与神经内分泌化有一定联系。3.分化程度越差,神经内分泌化的前列腺组织越多,这说明神经内分泌化与前列腺癌的分化和发展有关。3.NED阳性者Ki-67阳性率明显高于NED阴性者,提示Ki-67与神经内分泌化有一定关联,在判断前列腺癌预后方面,有一定指导意义。
