高密度脂蛋白胆固醇与心血管、肾脏功能关系的研究

【摘要】:背景:高密度脂蛋白胆固醇(HDL-C)作为一个重要的心血管危险因素,近年来被发现是评价中晚期慢性肾脏病患者心血管疾病负担的一个可靠指标。但一般人群中HDL-C与肾功能之间的关系尚不清楚。动脉僵硬度作为肾病患者的危险因素,在其具有更强预测意义的老年人群中与HDL-C的关系仍不明确。探讨HDL-C与动脉僵硬度的关系可以用来解释HDL-C与肾功能之间可能存在的相关性,但不能确定因果联系。D-4F是近来被广泛关注的载脂蛋白A-I模拟肽,具有生产成本低、可以口服等优点。D-4F作为干预措施,可以明确HDL-C与心血管、肾脏功能之间的因果关系。 方法:在4925名社区人群中对HDL-C与估计肾小球滤过率(eGFR)不同的四分位数之间的相关性进行单因素分析以及多元线性分析;研究780名社区老年人群动脉僵硬度的两个指标,颈-股动脉脉搏波速度(PWV)和颈-桡动脉PWV与HDL-C等危险因素之间的关系,对结果中同型半胱氨酸与HDL-C及动脉僵硬度分别的关系也进行了多元线性回归分析;用不同剂量D-4F干预长期高胆固醇饮食的Ldlr-/1小鼠,利用超声生物显微镜检测活体小鼠心脏结构与功能,再取小鼠心、肾标本切片染色,利用相关软件行图像分析计数。所有数据均以x±s表示,采用SAS统计软件进行相应的统计学分析。 结果: 1、调整混杂因素后,一般人群HDL-C水平独立地与eGFR所有四分位数相关联; 2、老年人群HDL-C水平独立地与主动脉僵硬度成反比,HDL-C与同型半胱氨酸水平呈负相关,同型半胱氨酸水平与主动脉僵硬度之间有关联; 3、高脂高胆固醇喂养的Ldlr-/-小鼠接受D-4F口服治疗后,体重出现剂量依赖性的减轻,心、肾组织中的脂类水平呈剂量依赖性改善,低剂量的D-4F就可以明显改善心脏结构及功能,D-4F对小鼠肾脏组织的保护作用随剂量增加而逐渐增强。 结论:在社区调查中发现低HDL-C水平与肾功能早期减退相关,可能部分与HDL对主动脉动脉僵硬度的保护作用减弱有关。在动物实验中观察到D-4F可以减轻高胆固醇饮食小鼠的体重,保护心、肾组织,证实HDL对心血管、肾脏的有益作用。
