椎基底动脉延长扩张症与后循环缺血和脑小血管病关联的机制

  背景椎基底动脉延长扩张症(Vertebrobasilar artery dolichoectasia,VBD)与脑小血管病(Cerebral small vessel disease,CSVD)有关,基质金属蛋白酶(Matrix metalloproteinases,MMPs)与组织金属蛋白酶抑制剂(Tissue inhibitor of metalloproteinases,TIMP)失衡可能是两者的共同机制。椎基底动脉形态学改变导致椎基底动脉系统血流动力学改变。一些研究报道了 VBD患者动脉血流缓慢。然而,应用灌注加权成像(Perfusion weighted imaging,PWI)确定的椎基底动脉供血区域灌注不足,以及应用PWI识别VBD引起的后循环缺血事件的报道较少。椎基底动脉扩张症(Vertebrobasilar ectasia,VBE)是VBD的一种主要类型,是脑卒中的预测因子,增加了死亡风险。然而,VBE与MMPs之间的关系尚不清楚。目的1.研究VBD与腔隙性梗死(Lacunar infarction,LI)、脑白质高信号(White matter hyperinten...
