从会话分析的角度研究中国医疗机构的医患风险交谈

  为了实现成功的医疗服务,医生与患者彼此之间的相互交流显得尤为重要。可以说,医患交际在医疗服务中起着关键作用。目前,研究者们也越来越重视对医患交际的研究。本文运用会话分析的手段对医患交际的其中一个方面——医疗风险交谈——进行探讨。本次研究所选语料均出自于对中国山西省内医疗机构的医生对患者的问诊过程的录音和转写。会话分析的研究对象是人们在交往活动中的谈话,它能更加直观和详细的展现人与人之间的会话活动。会话分析的主要概念包括话轮转换、话轮设计、序列结构和话题组织等。经过对中国医疗机构中的医患风险交谈进行认真分析,本研究主要从以下三个方面进行了探讨。首先,参照的标准不同,医疗风险交谈可有不同的分类。如:根据交谈中是否谈到利益可将医疗风险分为指定风险、均衡风险、对比风险和风险差异;根据风险交谈主题发起人的不同可将医疗风险交谈划分为由医生发起的医疗风险交谈和由患者发起的医疗风险交谈;根据交谈效果的不同又可将医疗风险交谈划分为产生积极效果的医疗风险交谈和产生消极效果的医疗风险交谈。其次,医患交谈中会出现就不同话题进行的交谈,医疗风险交谈作为其中的一个话题必然有其组织的规律,话题组织就包括有对话题提出...
