【摘要】:目的探讨应用双极射频电极通过三角形布针及复合三角形布针射频消融技术(RFA)对离体牛肝消融坏死区容积及扩大坏死消融的可行性及有效性,并为临床大肿瘤的扩大坏死性消融提供理论依据。方法采用Olympus celon双极消融针及其主机对离体牛肝进行消融,射频消融针的规格型号为T4.0,长度20cm,主机最大功率为250W/60Ω、470kHz、100-240V,50-60赫兹。1、三角形布针消融模式实验1:T4.0射频消融针、长度15cm,等距离三角形布针,针距3cm。实验2:等距离三角形布针,针距4cm。实验3:等距离三角形布针,针距5cm。2、复合三角形布针消融模式实验4:复合三角形布针,在离体牛肝插入5根射频针,使X、Y=6cm,在X、Y轴线上的3根射频针距离相等为3cm,等腰三角形另一边为4.2cm,根据消融顺序分为A、B、C、D四个区域,消融范围为A+B+C+D区域的叠加。实验5:复合三角形布针,在离体牛肝插入5根射频针,使X、Y=8,在X、Y轴线上的3根射频针距离相等为4cm,等腰三角形另一边为5.7cm,根据消融顺序分为A、B、C、D四个区域,消融范围为A+B+C+D区域的叠加。实验6:复合三角形布针,创建6个消融区,均为等边三角形布针,针距4cm在离体牛肝插入7根射频针,使X、Y=8,在X、Y轴线上的3根射频针距离相等为4cm,等腰三角形另一边为5.7cm,根据消融顺序分为A、B、C、D四个区域,消融范围为A+B+C+D+E+F区域的叠加。结果1、三角形布针消融模式实验1:应用Oiympus celonT4.0射频针,等距离三角形布针,针距为3cm,可获得毁损直径分别为X=5cm,Y=5cm,Z=5cm,消融容积65.41 cm3。实验2:等距离三角形布针,针距4cm。毁损直径X=6cm,Y=6cm,Z=6cm,消融容积113.03cm3实验3:等距离三角形布针,针距5cm,毁损直径X=7cm,Y=5cm,Z=4cm,消融容积73.26cm32、复合三角形布针消融模式实验4:复合三角形布针,在X和Y轴直径=6cm创建4个三角形消融地区,毁损直径X=7cm,Y=7 cm,Z=5 cm,消融容积128.21 cm3。实验5:复合三角形布针,4个三角形消融地区,X、Y轴直径为8cm,毁损直径X=8cm,Y=8cm,Z=7 cm,消融容积234.45 cm3。实验6:复合三角形布针,创建6个消融区,毁损直径X=9cm,Y=7cm,Z=7 cm,消融容积230.79 cm3。结论三角形布针消融模式,等距离布针,针距为3、4cm时,消融容积为球体,针距为4cm消融容积明显大于针距为3cm时消融容积;针距5cm,消融容积为类球体仅稍大于针距为3cm的消融容积,但消融灶周边融合不佳。复合三角形布针消融模式,4区消融模式X和Y轴直径8cm的消融容积明显大于直径6cm消融容积。6区消融模式的消融容积稍小于4区消融模式X和Y轴直径8cm的消融容积。应用4区和6区消融模式可明显的扩大消融范围和消融容积,对于大肿瘤的完全消融提供了有力的理论依据;6区消融模式消融容积小于4区消融模式X和Y轴直径8cm的消融容积,对于减少患者创伤和费用有重要的临床意义。临床消融模式的确定,要根据肿瘤的大小和形态,采取不同的消融模式,X、Y轴针距,该设备应不大于4cm。由于受肋间隙的影响,布针情况不可能达到实验的理想状态。
[Abstract]:Objective to investigate the application of bipolar radiofrequency electrode by triangular needle and needle cloth composite triangular radiofrequency ablation (RFA) on isolated bovine liver ablation necrosis volume and expand the feasibility and effectiveness of ablation and necrosis, to expand the clinical tumor necrotic ablation and provide a theoretical basis for the. Methods the Olympus celon bipolar ablation needle and its host were used to ablation the isolated bovine liver. The specification of the radiofrequency ablation needle is T4.0, and the length is 20cm. The maximum power of the host is 250W/60 ohm, 470kHz and 100-240V, and 50-60 Hz. 1. The triangle cloth needle ablation model experiment 1:T4.0 radiofrequency ablation needle, length 15cm, equal distance triangle cloth needle, needle distance 3cm. Experiment 2: the equal distance triangle cloth needle, the needle distance 4cm. Experiment 3: the equal distance triangle cloth needle, the needle distance 5cm. 2, composite triangular needle ablation model experiment 4: composite triangular needle, in vitro bovine liver inserted 5 RF needle, X, Y=6cm, 3 X, RF needle distance is equal in the Y axis is 3cm, an isosceles triangle on the other side of 4.2cm, according to the order of ablation is divided into A, B, C, D four regions, as the superposition of the A+B+C+D region of the ablation range. Experiment 5: composite triangular needle, in vitro bovine liver inserted 5 RF needle, X, Y=8, 3 X, RF needle distance is equal in the Y axis is 4cm, an isosceles triangle on the other side of 5.7cm, according to the order of ablation is divided into A, B, C, D four regions, ablation range of superposition of A+B+C+D region. Experiment 6: composite triangular needle, create 6 ablation zone, is an equilateral triangle cloth needle, needle distance 4cm in vitro bovine liver inserted 7 RF needle, X, Y=8, 3 X, RF needle distance is equal in the Y axis is 4cm, an isosceles triangle on the other side of 5.7cm, according to the ablation sequence is divided into A, B, C, D four, A+B+C+D+E+F area to overlay the ablation range. Results 1, triangle ablation mode experiment 1: using Oiympus celonT4.0 radiofrequency needle, equal distance triangular cloth needle and needle distance of 3cm, the diameter of damage can be X=5cm, Y=5cm, Z=5cm and ablation volume 65.41 cm3 respectively. Experiment 2: the equal distance triangle cloth needle, the needle distance 4cm. Damage to X=6cm in diameter, Y=6cm, Z=6cm, 113.03cm3 3: experimental ablation volume distance of triangular needle, needle distance 5cm, damage Y=5cm, diameter X=7cm, Z=4cm, 73.26cm32, composite volume ablation of triangular needle ablation model experiment 4: composite triangular needle, creating 4 triangles in the X and Y axis diameter =6cm ablation area. Damage to Y=7 cm, diameter X=7cm, Z=5 cm, the ablation volume 128.21 cm3. Experiment 5: Compound triangle needle, 4 triangle ablation area, X, Y axis diameter 8cm, damage diameter X=8cm, Y=8cm, Z=7 cm, ablation volume 234.45 cm3. Experiment 6: a compound triangle needle was used to create 6 ablation areas, which damaged the diameter X=9cm, Y=7cm, Z=7 cm, and the ablation volume was 230.79 cm3. Conclusion the triangular needle ablation mode, such as distance cloth needle, the needle distance is 3 4cm, when the ablation volume of sphere, needle distance of 4cm volume was significantly greater than the needle ablation distance of 3cm ablation volume; needle distance 5cm, ablation volume of a spheroid is only slightly larger than the needle distance of 3cm ablation volume, but ablation the surrounding poor integration. The ablation volume of the 4 zone ablation mode X and the Y axis diameter 8cm was significantly greater than the diameter of the diameter 6cm ablation. The ablation volume of the 6 area ablation mode was slightly smaller than that of the 4 zone ablation mode X and the Y axis diameter 8cm. Application of 4 zone and 6 zone ablation mode can significantly expand the scope of ablation and ablation volume, provides a strong theoretical basis for the complete ablation of large tumors; 6 ablation mode ablation volume less than 4 ablation mode X and Y axis diameter 8cm ablation volume for patients has important clinical significance to reduce trauma and cost. According to the size and shape of the tumor, we should take different ablation modes according to the size and shape of the tumor. The needle distance of X and Y axis should not be greater than 4cm. Because of the influence of the ribbed gap, the situation of the needle cloth can not reach the ideal state of the experiment.
