【摘要】：目的:通过Meta分析T1a声门型喉癌经激光手术和放射治疗后的保喉率、局部控制率、总生存率以及疾病特异性生存率。方法:检索医学数据库:PubMed、Web of Science、Embase数据库、Cochrane、中国生物文献数据库(CBM)、相关期刊论文(CNKI)、万方数字期刊全文数据库等,检索时限均为建库到2016年10月。由2位共同作者同时对搜索的相关文献进行阅读、筛选以及文献质量评价,从中提取与研究目的相关的详细数据,然后对提取的数据采用Review Manager 5.3软件进行Meta分析。结果:共纳入符合要求的文献12篇,总共1 530名研究对象。Meta分析结果表明,在对T1a声门型喉癌经激光手术和放射治疗后,在保喉率(OR=3.23,95%CI=1.21~8.60,P=0.02)方面,放疗要优于激光手术,在局部控制率、总生存率以及特异性疾病生存率方面未见明显统计学差异。结论:在对T1a声门型喉癌患者治疗中,放疗在保喉率方面优于激光手术,局部控制率、总生存率以及特异性疾病生存率未见明显差异。
[Abstract]:Objective: to analyze the laryngeal preservation rate and local control rate of T1a glottic carcinoma after laser surgery and radiotherapy by Meta. Methods: the general survival rate and disease-specific survival rate were analyzed. Methods: the medical database: the Web of Science of Science Embase database was searched. China Biological Literature Database (CBM), China Journal Full-text Database (CNKI), Wanfang Digital Journal Full-text Database, etc. The time limit of retrieval is from the establishment of the database to October 2016. The two co-authors simultaneously read, screen and evaluate the relevant documents searched, and extract the detailed data related to the purpose of the research. Then the extracted data were analyzed by Meta with Review Manager 5.3 software. Results: 12 papers were included. Meta-analysis of a total of 1 530 subjects showed that after laser surgery and radiotherapy for T1a glottic laryngocarcinoma, the laryngeal preservation rate was 3.23. 95 CI1. 21 ~ 8. 60% P0. 02) radiotherapy is better than laser surgery in local control rate. There was no significant difference in the overall survival rate and the survival rate of specific diseases. Conclusion: in the treatment of T1a glottic carcinoma, radiotherapy is superior to laser surgery in laryngeal preservation rate and local control rate. There was no significant difference in overall survival rate and specific disease survival rate.
