【摘要】:目的目前,癌症已经成为人类健康的第一杀手,尤其是肝细胞肝癌(Hepatocellular carcinoma, HCC),在中国甚至世界范围内,发病率和死亡率在逐年升高。因此,寻找新的诊断和治疗相关分子协助临床医生快速有效地诊断和治疗HCC将有利于提高HCC病人的疗效和生存率。环氧合酶(cyclooxygenase, COX)在体内有着重要的作用,近年来的研究表明,Ⅱ型环氧合酶(COX-2)在食管癌、肝癌中表达增高,与肿瘤血管的形成密切相关。CD88又称补体5片段受体(complement component 5a receptor, C5aR),常被用来作为鉴定白细胞系的标志,与C5a结合后能够激活PI3K/A KT/mTOR和IL-6/STAT3两个信号通路并促进肝细胞的分裂增殖和对抗凋亡,趋化白细胞和参与炎症反应,在肿瘤侵袭转移中发挥重要作用。已有研究显示CD88可能促进肿瘤周围血管新生,加速肿瘤细胞侵袭转移。由于COX-2和CD88在肿瘤组织中的表达均显著增高,并且与肿瘤的侵袭转移密切相关,因此,本研究将探讨二者在肝细胞肝癌组织中的表达率以及二者之间的相关性。方法1.标本收集收集新乡市第一人民医院病理科2013年1月至2014年4月期间,所有资料完整的肝细胞肝癌组织共57例。根据年龄、性别、肿瘤直径、分化程度、分期和有无淋巴结转移等将标本分类。一部分组织放入液氮保存,另一部分组织经10%的福尔马林溶液固定、石蜡包埋以及4gm厚度连续切片。2. RT-PCR检测正常肝组织和肝细胞肝癌组织中COX-2和CD88mRNA的表达提取肝正常组织和肝细胞肝细胞肝癌组织中总mRNA,检测组织中COX-2和CD88 mRNA的表达情况。3.免疫组织化学染色观察COX-2和CD88蛋白的表达将准备好的组织切片用SABC法进行免疫组织化学染色,观察并确定COX-2和CD88的表达部位,计算二者的表达率以及表达的相关性。4.结果判断RT-PCR的结果用Image J软件测量灰度值,根据与内参的对比,判断COX-2和CD88 mRNA的表达。免疫组织化学结果根据染色强度和阳性细胞所占的百分比分别计分,再将染色强度分值与阳性细胞所占百分比分值相乘所得乘积判定染色结果。5.统计学处理计量数据均使用均数±标准差(x±s)表示,应用SPSS 17.0统计软件对数据进行统计学处理。多样本均数的比较采用方差分析;阳性率的比较用χ2检验;相关性分析用Pearson检验,检验水准均设为α=0.05,即P0.05有显著统计学差异,P0.01有非常显著统计学差异,P0.05无统计学差异。结果1.COX-2和CD88 mRNA的表达COX-2和CD88 mRNA在正常肝组织和肝细胞肝癌组织中均有表达,且在肝细胞肝癌组织的表达较正常肝组织明显增高。二者在低、中、高分化癌组织中表达逐渐增高,低、中分化组表达无统计学差异,而二者与高分化组相比有统计学差异(P0.05)。2.COX-2在肝细胞癌中的表达及与临床病理特征的关系COX-2染色阳性的细胞其细胞浆中呈现弥漫性分布的黄色和棕黄色颗粒状物质,肝细胞癌组织中的COX-2阳性率为73.68%。COX-2蛋白的表达与肿瘤分化程度、肿瘤大小、临床分期、淋巴结转移情况无关(P0.05)。3.CD88在肝细胞癌中的表达及与临床病理特征的关系CD88蛋白表达主要位于细胞膜,部分位于胞浆中。肝细胞癌组织中的CD88蛋白表达的阳性率为82.45%。CD88蛋白的表达与肿瘤分化程度、肿瘤大小、临床分期、淋巴结转移情况无关(P0.05)3.COX-2与CD88在肝细胞癌中的表达的相关性肝癌组织中,COX-2阳性组,CD88的阳性率为90.48%,显著高于COX-2阴性组(60%),COX-2和CD88的蛋白表达呈正相关(r=0.353,P0.05)。结论1.肝细胞肝癌组织中COX-2和CD88 mRNA的表达较正常肝组织明显增多,但COX-2和CD88蛋白的表达与肿瘤分化程度、肿瘤大小、临床分期和淋巴结转移均无关。2.肝细胞肝癌组织中COX-2和CD88蛋白的表达呈正相关。
