3D与2D腹腔镜胃癌根治术近期疗效对比研究

【摘要】：目的对比分析3D和2D腹腔镜胃癌根治术的临床疗效。方法回顾性分析2015年11月至2016年11月解放军总医院普通外科同一手术组实施腹腔镜手术治疗的156例胃癌病人临床资料,其中行3D腹腔镜胃癌根治术70例(3D组),2D腹腔镜胃癌根治术86例(2D组),比较两组病人术中及术后相关指标。结果与2D组相比,3D组病人手术时间更短[(232.9±95.8)min vs.(202.4±89.7)min,P=0.044],术中出血更少[(152.8±41.0)m L vs.(138.9±42.0)m L,P=0.039]。在淋巴结清扫数、阳性淋巴结数、术后排气时间、住院时间、住院费用、术后并发症发生率及严重程度方面,组间差异均无统计学意义(P0.05)。结论 3D腹腔镜胃癌根治术能获得与2D腹腔镜手术相近的临床短期疗效,同时缩短了手术时间和减少了术中出血。
[Abstract]:Objective to compare the clinical results of 3D and 2D laparoscopic radical gastrectomy for gastric cancer. Methods from November 2015 to November 2016, the clinical data of 156 patients with gastric cancer treated by laparoscopic surgery in the same operation group in General surgery Department of PLA General Hospital were retrospectively analyzed. Among them, 70 cases underwent 3D laparoscopic radical gastrectomy and 86 cases underwent 2D laparoscopic radical gastrectomy in 3D group. The operative and postoperative parameters were compared between the two groups. Results compared with 2D group, the operative time of 3D group was shorter [232.9 卤95.8min vs.(202.4 卤89.7min P0. 044], and the bleeding was more severe during operation. The number of lymph node dissection was 152.8 卤41.0mL vs.(138.9 卤42.0mL P0. 039. The number of positive lymph nodes, postoperative exhaust time, hospitalization time, hospitalization cost, incidence of postoperative complications and severity, Conclusion 3D laparoscopic radical resection of gastric cancer is similar to that of 2D laparoscopic surgery, and it can shorten the operation time and reduce intraoperative bleeding.
