足趾甲下外生骨疣的分型与手术切口选择策略

【摘要】：[目的]探讨足趾甲下外生骨疣的分型与手术切口选择策略,分析甲床处理的方法。[方法]2008年1月~2013年12月,本院收治的足趾甲下外生骨疣患者共计23例,根据体检与足部正、侧位X线片,判断病灶部位与甲床的关系,分型为I型与II型。采取不同切口与甲床处理方式,随访终点记录患者趾甲生长情况与患者满意度。[结果]23例患者,男7例,女16例。左足9例,右足14例。平均年龄(24.4±11.9)岁(13~56岁),病程14.0个月(2~108个月)。随访时间18~61个月,平均24.2个月。病灶均位于趾骨末节,鐷母趾19例,第2趾1例,第3趾1例,第4趾2例。分型为IA 2例,IB与IC 6例,IIA 7例,IIB 6例,第4趾末节趾甲床成形2例。[结论]足趾甲下外生骨疣,根据病灶部位与甲床的关系,选择合适切口与甲床处理方式,有助于提高优良率。
[Abstract]:[objective] to study the classification and surgical incision selection strategy of exophytic verruca under toe nail, and to analyze the treatment methods of nail bed. [methods] from January 2008 to December 2013, 23 patients with exophytic verruca under toe were treated in our hospital. Lateral radiographs were used to determine the relationship between the location of the lesion and the nail bed, which were classified as type I and type II. Different incision and nail bed treatments were used to record the growth of toenail and patient satisfaction at the end of follow up. [results] there were 23 patients (7 males). 16 cases were female, 9 cases were left foot, 14 cases were right foot. The mean age was 24.4 卤11.9years old 1356 years old, the course of disease was 14.0 months or 2 ~ 108 months. The follow-up time was 18 ~ 61 months (mean 24.2 months). The lesions were located in the distal phalangeal ganglion, the female toe in 19 cases, the second toe in 1 case, the third toe in 1 case. 2 cases of the fourth toe were classified as IA (2 cases) and IC (6 cases), IIA (7 cases), IIB (6 cases), toenail bed formation (2 cases). [conclusion] according to the relationship between the location of the lesion and the nail bed, appropriate incision and nail bed treatment were selected. It is helpful to improve the excellent and good rate.
