直肠腔内超声与磁共振对直肠癌术前分期的评价及临床应用价值探讨

【摘要】：目的:评估直肠腔内超声(Endorectal Ultrasound,ERUS)、磁共振(Magnetic resonance imaging MRI)对直肠癌患者术前分期与病检对照的准确性。方法:回顾性分析2015年1月至2015年12就诊于我院行经直肠腔内超声检查和盆腔磁共振检查的72例直肠癌患者,根据术后病检结果,按照美国癌症联合委员会(American Joint Committee On Cancer,AJCC)直肠癌分期指南行T、N分期,将直肠腔内超声和磁共振对直肠癌局部T、N分期的准确性与病检结果对照,并行一致性评价。结果:总共纳入72例直肠癌患者,与术后病检结果对照,经直肠腔内超声、磁共振的T分期总准确率分别为81.94%(59/72)、66.67%(48/72)(Kappa值分别为0.745,0.524,P均0.001),经直肠腔内超声、磁共振的N分期总准确率分别为65.28%(47/72)、77.78%(56/72)(Kappa值分别为0.350、0.599,P均0.001);结论:与病检结果对照,直肠腔内超声对直肠癌术前T分期的一致性较好,而磁共振对N分期的一致性较好,直肠腔内超声与磁共振是评估直肠癌术前分期有效的影像学检查方法。
[Abstract]:Objective: To evaluate the transrectal ultrasound (Endorectal Ultrasound, ERUS), magnetic resonance (Magnetic resonance imaging MRI) on the accuracy of preoperative staging and pathological examination. Methods: a retrospective analysis from January 2015 to 2015 12 patients in our hospital underwent transrectal ultrasonography and Pelvic MRI of 72 cases of colorectal cancer patients. According to the postoperative pathological results, according to the American Joint Committee on cancer (American Joint Committee On Cancer, AJCC) for T staging, N staging, the transrectal ultrasound and magnetic resonance imaging of rectal cancer Department of the Ministry of T, N and pathological results of the control accuracy of staging, parallel consistency evaluation. Results: a total of 72 cases were included in patients with rectal cancer, compared with pathological results after surgery, transrectal ultrasound, magnetic resonance imaging and the total accuracy of T was 81.94% (59/72), 66.67% (48/72) (Kappa = 0.745,0.524, P < 0.001), the Transrectal ultrasound, magnetic resonance imaging and the total accuracy of N was 65.28% (47/72), 77.78% (56/72) (Kappa = 0.350,0.599, P < 0.001); conclusion: compared with pathological results, transrectal ultrasonography in the preoperative staging of rectal cancer T consistency is good, and the magnetic resonance of N staging is better, transrectal ultrasonography and magnetic resonance imaging is to assess the preoperative staging of rectal cancer effective imaging method.
