Nanog、Oct4在宫颈癌中的表达与临床意义

【摘要】:目的宫颈癌组织中Nanog与Oct4表达及其临床意义。方法选取江苏省淮安市妇幼保健院妇科2015年5月-2016年4月期间收治的宫颈癌患者40例作为观察组研究对象,选取同期我院收治的宫颈上皮内瘤变III级(cervical intraepithelial neoplasia,CIN III)患者30例,因子宫肌瘤患者行子宫切除术获得的正常宫颈组织10例为研究对照组。采用SP免疫组化染色法检测宫颈癌组织、CINIII组织及正常宫颈组织中Nanog、Oct4的表达情况,分析宫颈癌组织中Nanog、Oct4表达与疾病临床病理特征间存在的关系,进行Nanog与Oct4表达之间相关性的分析。结果(1)免疫组化染色法检测结果显示,Nanog与Oct4在宫颈组织中均有表达,Nanog阳性表达呈现点状,在细胞核上少有灶状聚集,多数存在于间质和癌巢交界的边缘。Oct4阳性表达则呈现棕黄色颗粒状,多表达于细胞核。通过对比发现,宫颈癌组织中Nanog与Oct4的阳性表达率分别为92.50%、92.50%,而CIN组织中阳性表达率分别为70.00%、76.67%,正常宫颈组织中的阳性表达率最低,仅为20.00%、20.00%,三组两两比较,均存在统计学差异(P0.05)。(2)Nanog表达量在宫颈癌组织、CIN组织及正常宫颈组织中分别为(49.57±15.24)×102、(32.31±4.85)×102及(8.46±1.28)×102,三者的表达量IOD比较存在统计学差异(P0.05);Oct4的表达量IOD在宫颈癌组织、CINIII组织及正常宫颈组织中分别为(15.84±3.25)、(10.39±2.98)及(1.35±0.52)三者的表达量IOD比较存在统计学差异(P0.05)。(3)探讨宫颈癌组织中Nanog与Oct4的表达情况与疾病病理参数之间的关系发现,Nanog与Oct4的表达水平同临床分期和淋巴结转移之间无相关性(P0.05);Nanog与Oct4的表达水平在宫颈癌高分化、低分化、中分化组织中存在显著性差异,两者的表达水平均与肿瘤的分化程度呈现负相关(P0.05)。(4)对宫颈癌组织中Oct4及Nanog表达情况采用Spearman等级相关分析,结果表明两者在宫颈癌组织中的表达呈现正相关性(rs=0.5315,P=0.0026)。结论在宫颈癌组织中,Nanog与Oct4均存在过量表达;Nanog与Oct4的表达水平与宫颈癌组织的分化程度呈现负相关,分化程度越高,表达水平越低;Nanog与Oct4的表达水平与宫颈癌组织临床分期和淋巴结转移无相关性;宫颈癌组织中Nanog与Oct4的高表达之间存在正相关关系。
[Abstract]:Objective to investigate the expression of Nanog and Oct4 in cervical carcinoma and its clinical significance. Methods 40 patients with cervical cancer admitted from May 2015 to April 2016 in Huaian City Maternal and Child Health Hospital in Jiangsu Province were selected as the observation group. Thirty patients with cervical intraepithelial neoplasia (III grade III cervical intraepithelial neoplasia neoplasia cin III) treated in our hospital during the same period were selected. The expression of Nanog-Oct4 in cervical carcinoma tissue (CINIII) and normal cervix cervix was detected by SP immunohistochemical staining. 10 cases of normal cervix cervix tissues obtained by hysterectomy were used as the control group, and the expression of Nanog-Oct4 in cervical carcinoma tissues was detected by SP immunohistochemical staining. To analyze the relationship between the expression of Nanog-Oct4 and the clinicopathological features of cervical cancer. The correlation between Nanog and Oct4 expression was analyzed. Results 1) Immunohistochemical staining showed that the positive expression of Nanog and Oct4 in cervical tissues were punctate, and there were few focal aggregations in the nucleus. The majority of the positive expression of .Oct4 at the edge of the stroma and the edge of the cancer nest was brown granular, mostly expressed in the nucleus. The positive expression rates of Nanog and Oct4 in cervical cancer tissues were 92.50 and 92.50, respectively, while those in CIN tissues were 70.000.76. 677.The positive rates of normal cervix tissues were the lowest, 20.000.00 and 20.000.00, respectively. There was statistical difference in the expression of P0.05, P0.05, P0. 05 ~ (2 +) Nanog in cin and normal cervix tissues, respectively, which were 49.57 卤15.24 脳 10 ~ 2 脳 10 ~ 2 脳 10 ~ 2 and 8.46 卤1.28 脳 10 ~ 2 脳 10 ~ 2, respectively. There was a significant difference in IOD expression of IOD in cervical carcinoma tissue and normal cervix. There was a statistical difference among them in the expression of P0.05OT4 in cervical carcinoma tissue and normal cervix tissue. The expression levels of IOD in cervical tissues were 15.84 卤3.25 (10.39 卤2.98) and 1.35 卤0.52 (respectively). There was a statistical difference between the expression of Nanog and Oct4 in cervical carcinoma tissues (P0.05) and the relationship between the expression of Nanog and Oct4 and the pathological parameters of cervical cancer. It was found that the expression levels of Nanog and Oct4 were related to the clinical stages of cervical cancer. There was no correlation between the expression of P0.05 and Oct4 and lymph node metastasis in cervical carcinoma. The expression of Oct4 and Nanog was negatively correlated with the degree of tumor differentiation. Spearman grade correlation analysis was used to analyze the expression of Oct4 and Nanog in cervical carcinoma. The results showed that there was a positive correlation between the expression of Oct4 and Oct4 in cervical carcinoma tissues. Conclusion the overexpression of Oct4 and Oct4 in cervical carcinoma tissues were negatively correlated with the differentiation degree of cervical carcinoma tissues, and the higher the degree of differentiation was, the higher the degree of differentiation was. There was no correlation between the expression level of Nanog and Oct4 and the clinical stage and lymph node metastasis of cervical cancer, but there was a positive correlation between the high expression of Nanog and Oct4 in cervical carcinoma.
