肺小结节胸腔镜术前医用胶定位的应用价值

【摘要】：目的研究肺小结节(SPN)胸腔镜术前医用胶定位的应用价值。方法回顾分析41例孤立性SPN胸腔镜术前定位病例资料,包括医用胶定位组22例,Hook-wire定位组19例。统计医用胶定位的定位效果、并发症、衔接期时间以及肺楔形切除所用时间,并将两组结果进行对比分析。结果两组病例定位均取得成功;医用胶组的气胸发生率(18.2%)、肺出血发生率(9.1%)、胸痛发生率(4.5%)均低于Hook-wire组,其中肺出血发生率与Hook-wire组比较差异有统计学意义(P0.01);医用胶组无并发症率(63.6%)高于Hook-wire组(21.1%),差异有统计学意义(P0.05);衔接期时间医用胶组(16.32±8.83)h长于Hook-wire组(3.29±4.21)h,差异有统计学意义(P0.01);作楔形切除所用时间医用胶组(21.14±7.01)min与Hook-wire组(18.58±5.22)min差异无统计学意义(P0.05)。结论 SPN胸腔镜术前医用胶定位安全可靠、效果良好,与Hook-wire定位比较并发症发生率更低,并可获得更长的衔接期,具有较高的应用价值。
[Abstract]:Objective to study the application value of medical glue localization before thoracoscopic surgery with SPN.Methods the data of 41 patients with solitary SPN before thoracoscopic localization were retrospectively analyzed, including 22 cases of medical glue localization group (22 cases) and 19 cases of Hook-wire localization group (19 cases).The localization effect, complications, the time of convergence and the time of wedge resection of lung were analyzed and compared between the two groups.Results the incidence of pneumothorax, pulmonary hemorrhage, chest pain and chest pain in the medical glue group were 18. 2%, 9. 1% and 4. 5%, respectively, which were lower than those in the Hook-wire group.The incidence of pulmonary hemorrhage in the medical glue group was significantly higher than that in the Hook-wire group (P 0.05), and in the medical glue group was 16.32 卤8.83 hours longer than that in the Hook-wire group (3.29 卤4.21 h), and the difference was statistically significant (P 0.01), and the incidence of pulmonary hemorrhage was significantly higher in the medical glue group than that in the Hook-wire group (P 0.01), and the incidence of pulmonary hemorrhage was significantly higher in the medical glue group than in the Hook-wire group (P < 0.01), while the incidence of pulmonary hemorrhage in the medical glue group was significantly higher than that in the Hook-wire group (P 0.05).The time used for wedge resection was 21.14 卤7.01)min in the medical glue group and 18.58 卤5.22)min in the Hook-wire group. There was no significant difference between the two groups (P 0.05).Conclusion the localization of medical glue before SPN thoracoscopy is safe, reliable and effective. Compared with Hook-wire, the incidence of complications is lower, and a longer connecting period can be obtained. It has a higher application value.
