SALL4和Bmi-1在宫颈癌组织中的表达及临床意义

【摘要】:目的探讨人婆罗双树样基因-4蛋白(Sal-like protein 4,SALL4)和B细胞特异的莫洛尼白血病毒插入点1基因蛋白(B-cell specific moloney leukemia virus insertion site 1,Bmi-1)的表达在宫颈癌发生发展过程中的意义。方法病例研究选取:患者住院时间2014年9月到2016年9月手术并确诊的宫颈癌、宫颈上皮内病变(CIN)患者以及正常健康宫颈组织。随机选取宫颈癌患者30例,其中鳞癌患者22例,腺癌患者8例,年龄25~69岁,中位年龄45岁。选取CIN患者60例,包括低级别CIN 30例和高级别CIN 30例,年龄22~68岁,中位年龄46岁。同时收集正常宫颈组织标本30例设置为对照组。本研究中所有的宫颈组织标本在患者术后均经过病理科病理确诊,且患者在术前均未接受放疗、化疗或任何激素治疗的病例。三组宫颈组织中的SALL4和Bmi-1表达水平采用免疫组织化学技术(SP法)检测。采用SPSS19.0统计软件进行数据分析,数据的集中趋势和离散趋势(`X±S)表示,P0.05作为差别具有统计学意义的标准。应用卡方检验分析各组之间所测指标的差异。Pearson法分析SALL4和Bmi-1表达水平的相关性。结果SALL4阳性表达率:正常宫颈组织、低级别宫颈上皮内病变、高级别宫颈上皮内病变和宫颈癌组织中的表达分别是10.00%、36.67%、63.33%、86.67%(P0.05);采用SPSS19.0统计软件进行组间差异应用卡方检验,差异存在统计学意义(P0.05)。宫颈癌中SALL4蛋白的表达水平显著高于其他三种宫颈组织(P0.05);低级别期的CIN组织中SALL4含量低于高级别期(P0.05);而正常宫颈组织中的SALL4表达水平在四种宫颈组织中表达水平最低(P0.05)。Bmi-1阳性表达率:正常宫颈组织、低级别宫颈上皮内病变、高级别宫颈上皮内病变和宫颈癌组织中的表达分别是6.67%、23.33%、46.67%、73.33%(P0.05);采用SPSS19.0统计软件进行组间卡方检验,差异存在统计学意义(p0.05)。宫颈癌中Bmi-1的表达水平明显高于CIN及正常宫颈组织(P0.05),高级别期CIN的Bmi-1表达明显高于低级别期组织(P0.05),而CIN中Bmi-1的表达亦显著高于正常宫颈组织(P0.05)。采用SPSS19.0统计软件对SALL4与Bmi-1 pearson相关性检验,呈正相关(r=0.798,P0.01)。SALL4和Bmi-1与宫颈癌临床病理特征关系:SALL4、Bmi-1蛋白表达水平与淋巴结转移(t=1.907,p0.05),组织学分级(t=0.993,p0.05)及临床分期(t=1.080,p0.05)相关,而与年龄(t=1.436,p0.05)、肿瘤大小(t=1.286,p0.05)均无关。SALL4、Bmi-1蛋白在40岁到59岁阳性率表达最高,但不具有统计学意义。临床分期越高SALL4、Bmi-1蛋白表达水平越高。有淋巴结转移的SALL4、Bmi-1蛋白表达明显高于无淋巴结转移的宫颈癌组织。结论:(1)在宫颈癌中SALL4和Bmi-1表达明显升高,且高于其他宫颈对照组织(低级别宫颈上皮内病变、高级别宫颈上皮内病变、和正常宫颈组织),SALL4和Bmi-1与CIN病变级别相关,病变级别越高,表达越高,癌变的程度越大。(2)在宫颈癌的发病过程中SALL4和Bmi-1变化具有平行性,提示二者具有协同作用。
[Abstract]:Objective to investigate the significance of expression of B-cell specific moloney leukemia virus insertion site 1 (B-cell specific moloney leukemia virus insertion site 1) in carcinogenesis and progression of cervical carcinoma.Methods patients with cervical cancer, cervical intraepithelial lesions (CINs) and normal cervical tissues were selected from September 2014 to September 2016.Thirty cases of cervical cancer were randomly selected, including 22 cases of squamous cell carcinoma and 8 cases of adenocarcinoma. The age was 2569 years old with a median age of 45 years.Sixty patients with CIN were selected, including 30 cases of low grade CIN and 30 cases of high grade CIN. The age was 22 ~ 68 years old with a median age of 46 years.At the same time, 30 cases of normal cervical tissue were collected as control group.All cervical tissue specimens in this study were confirmed by pathology after operation and none of them received radiotherapy chemotherapy or any hormone therapy before operation.The expression of SALL4 and Bmi-1 in cervical tissues of the three groups were detected by immunohistochemical technique and SP method.SPSS19.0 statistical software was used to analyze the data. The data concentration trend and discrete trend (`X 卤S) indicated that P0.05 was regarded as the standard of statistical significance.The correlation between the expression of SALL4 and Bmi-1 was analyzed by chi-square test.Results the positive expression rate of SALL4 in normal cervix, low grade cervical intraepithelial lesion, high grade cervical intraepithelial lesion and cervical carcinoma was 10.00 ~ 36.67 ~ 36.67 ~ (33.33) ~ 86.67 ~ (th) P 0.05 respectively.The difference was statistically significant (P 0.05).The expression of SALL4 protein in cervical cancer was significantly higher than that in the other three cervical tissues (P0.05A), the SALL4 level in low-grade CIN tissues was lower than that in high-grade CIN tissues, and the SALL4 expression level in normal cervix tissues was higher in the four cervical tissues.The lowest positive rate of P0.05.
