化疗干预对恶性肿瘤患者外周血单个核细胞功能的影响

【摘要】:恶性肿瘤的发生、发展与增殖有关。恶性肿瘤细胞寄生在人体,机体是其赖以生存的内环境。因此,研究外周血单个核细胞的改变有可能为进一步探讨恶性肿瘤细胞与机体之间关系提供理论依据。外周血单个核细胞,主要包括淋巴细胞及单核细胞,淋巴细胞包括T淋巴细胞、B淋巴细胞及自然杀伤细胞,在免疫监视、杀伤靶细胞及免疫调节方面具有极其重要的作用,是机体抗肿瘤免疫的重要效应细胞,外周血单个核细胞的数量及功能状态反映机体全身免疫机能水平。IL-8是多细胞来源、多功能的细胞因子,能促进炎性反应、刺激血管生成、促进有丝分裂、调节宿主免疫功能等作用,在多种肿瘤中高表达,与肿瘤发生的早期事件及肿瘤的进展密切相关。流式细胞术是一种在功能水平上对单细胞或其他生物粒子进行定量分析和分选的检测手段,是当前检测细胞分化及功能的最先进、最准确的技术,主要应用在癌前病变及肿瘤的早期诊断、交叉配型、测定骨髓造血干细胞的重建、淋巴细胞亚群分析、DNA细胞分化和周期的分析及细胞凋亡等方面的检测。化疗是恶性肿瘤的主要治疗手段之一,除对恶性肿瘤细胞有杀伤作用,对机体正常组织细胞亦有损伤。目的:探讨化疗干预对恶性肿瘤患者外周血单个核细胞功能影响。方法:应用流式细胞术,对43例恶性肿瘤患者和10例体检正常健康者晨空腹外周肘静脉血进行外周血单个核细胞细胞周期、凋亡率及IL-8的检测。结果:1正常健康者与恶性肿瘤患者化疗前两组外周血单个核细胞功能之间比较:G0/G1期比例减低,S期比例升高,G2/M期比例升高,凋亡率升高,IL-8增高,差别均无统计学意义(P0.05)。2正常健康者与恶性肿瘤患者化疗后两组外周血单个核细胞功能之间比较:1)恶性肿瘤患者化疗后较正常健康者G0/G1比例下降(86.06士9.08):(93.18士4.07),G2/M期比例升高(11.51士8.27):(4.00士2.24),差别有统计学意义(P0.05)。2)S期比例升高,IL-8比例增加,化疗后较正常健康者外周血单个核细胞凋亡率增加,但差别均无统计学意义(P0.05)。3正常健康者与恶性肿瘤小负荷组化疗后两组之间外周血单个核细胞功能比较:1)化疗后与正常健康者比较G0/G1比例下降(85.64士9.85):(93.18士4.07),G2/M期比例升高(12.11士8.45):(4.00士2.24),凋亡率增加(16.90士18.24):(2.90士1.87),差别有统计学意义(P0.05)。2)S期比例增加,IL-8比例增加,但差别无统计学意义(P0.05)。4正常健康者与恶性肿瘤大负荷组化疗后两组之间外周血单个核细胞功能比较。1)化疗后与正常健康者比较G0/G1比例下降(86.62士8.33):(93.18士4.07),G2/M期比例升高(10.71士8.33):(4.00士2.24),差别有统计学意义(P0.05)。2)S期比例减低,IL-8比例增加,外周血单个核细胞凋亡率增加,但差别无统计学意义(P0.05)。5恶性肿瘤患者化疗前后两组之间外周血单个核细胞功能比较:1)化疗后较化疗前G0/G1比例下降,S期比例降低,G2/M期比例升高,IL-8比例减低,差别无统计学意义(P0.05)。2)化疗后较化疗前外周血单个核细胞凋亡率增加(12.63士18.09):(4.68士8.88),有统计学意义(P0.05)。6恶性肿瘤小负荷组患者化疗前后两组之间外周血单个核细胞功能比较。1)化疗后较化疗前G0/G1比例下降,S期比例下降,G2/M期比例升高,IL-8比例增加,但差别无统计学意义(P0.05)。2)化疗后较化疗前外周血单个核细胞凋亡率增加(16.90士18.24):(4.45士7.21),差别有统计学意义(P0.05)。7恶性肿瘤大负荷组患者化疗前后两组之间外周血单个核细胞功能比较。化疗后较化疗前G0/G1比例下降,S期比例增加,G2/M期比例升高,外周血单个核细胞凋亡率增加,IL-8比例减低,差别无统计学意义(P0.05)。8恶性肿瘤患者按照肿瘤负荷分亚组后两组化疗前外周血单个核细胞功能比较:化疗前恶性肿瘤大负荷组较小负荷组IL-8比例增加(4.54±3.83):(2.06±2.14),差别有统计学意义(P0.05)。G0/G1比例增高,S期比例降低,G2/M期比例增高,凋亡率增加,均无统计学差异(P0.05)。9恶性肿瘤患者按照肿瘤负荷分亚组后两组化疗后外周血单个核细胞功能比较:化疗后恶性肿瘤大负荷组较小负荷组G0/G1比例增加、S期比例减低、G2/M期比例减低、凋亡率减低及IL-8比例增加,均无统计学差异(P0.05)。结论:1恶性肿瘤化疗前患者与正常健康者之间在细胞周期、凋亡率及IL-8水平均无统计学差异。2恶性肿瘤化疗后患者与正常健康者之间在细胞周期上,较正常健康者G0/G1期比例减低,G2/M期比例升高。3化疗干预后可增高外周血单个核细胞凋亡率,建议科研抽血时间为化疗前。4 IL-8在化疗前、肿瘤大负荷组明显增高,表明IL-8更能反映恶性肿瘤患者外周血单个核细胞功能。5化疗可明显改变恶性肿瘤患者外周血单个核细胞细胞周期和凋亡率,为后续研究样本抽血时间、免疫功能检测提供理论依据。

Chemotherapy is the main treatment in malignant tumors, except has the function of killing tumor cells, also have damage to the body the normal tissue cells. Objective: To investigate the effect of chemotherapy for patients with malignant tumors of peripheral blood mononuclear cells. Methods: using flow cytometry in 43 cases of malignant tumor patients and 10 cases of normal health examination Morning fasting peripheral venous blood of peripheral blood mononuclear cell nuclear cell cycle, apoptosis rate and IL-8.
