宫腔镜保守性手术联合孕激素对早期子宫内膜癌保留生育功能治疗的Meta分析

【摘要】：目的:本文运用Meta分析的方法,对年轻早期子宫内膜癌患者应用宫腔镜保守性手术联合孕激素治疗的疗效、治疗后妊娠等相关问题进行讨论分析。方法:收集2000年1月至2017年3月之间发表的关于40岁以下早期子宫内膜癌患者应用宫腔镜保守性手术联合孕激素治疗以保留生育功能的相关文献,严格按照纳入标准选择文献,纳入原始的研究资料,提取资料及有效数据,利用R软件(R3.3.2)进行单个样本率的Meta分析。结果:共15篇文献入选,203例患者参与研究。Meta分析结果示:宫腔镜保守性手术联合孕激素治疗后的完全缓解率为89.40%,95%CI[0.8329-0.9346],复发率为10.90%,95%CI[0.0682-0.1741],总妊娠率为63.44%,95%CI[0.5572-0.7053],总分娩率为59.69%,95%CI[0.5262-0.6656]。结论:多数早期子宫内膜癌患者应用宫腔镜保守性手术联合孕激素治疗可获得完全缓解,并顺利妊娠、分娩,部分患者存在复发或治疗无效的情况,但及时接受传统手术治疗,仍能获得良好预后。
[Abstract]:Objective: to analyze the effects of hysteroscopic conservative surgery combined with progesterone on the treatment of early endometrial carcinoma in young patients with early endometrial carcinoma by means of Meta analysis.Methods: from January 2000 to March 2017, the literature on hysteroscopic conservative surgery combined with progesterone therapy to preserve fertility function in patients with early endometrial carcinoma under 40 years old was collected and selected according to the inclusion criteria.The Meta analysis of single sample rate was carried out by using R software R3.3.2), which included the original research data, extracted the data and valid data.Results: the results of Meta-analysis showed that the complete remission rate of hysteroscopic conservative surgery combined with progesterone therapy was 89.400.95 CI [0.8329-0.9346], the recurrence rate was 10.90 ~ 95CI [0.0682-0.1741], the total pregnancy rate was 63.4495 CI [0.5572-0.7053], the total delivery rate was 59.699595 CI [0.5262-0.6656].Conclusion: most patients with early endometrial carcinoma can be treated with hysteroscopic conservative surgery combined with progesterone for complete remission, pregnancy, delivery, recurrence or ineffective treatment in some patients, but receive traditional surgical treatment in time.A good prognosis can still be obtained.
