血浆中p16及CDO1基因甲基化联合检测在胶质瘤诊断中的意义

【摘要】:目的探讨血浆中联合检测p16及CDO1基因甲基化在胶质瘤诊断中的意义。方法采用甲基化特异PCR(Methylation-specific PCR,MSP)法分别检测49例胶质瘤患者肿瘤组织及其对应的术前血浆、10例颅脑外伤患者挫伤的脑组织及其对应的术前血浆和16例健康体检者血浆中p16及半胱氨酸双加氧酶1(cysteinedioxygenase1,CDO1)基因的甲基化状态,以探讨血浆中检测p16及CDO1基因甲基化在胶质瘤临床诊断中的价值。结果胶质瘤组织中p16及CDO1基因甲基化率分别为53.1%(26/49)、42.9%(21/49),对应的血浆甲基化率为39.0%(19/49)、34.7%(17/49)。在肿瘤组织和血浆中p16及CDO1至少一个(p16和/或CDO1)基因甲基化的频率为75.5%(37/49)和67.3%(33/49)。血浆中p16及CDO1基因(甲基化)联合检测胶质瘤的灵敏度为67.3%,较单基因检测的灵敏度明显提高。对照组中10例脑挫伤组织和26例血浆中均未检测出p16和CDO1基因甲基化异常,提示p16及CDO1基因(甲基化)单独或联合检测的特异度都为100%。单因素分析显示术前患者血浆中p16和/或CDO1基因甲基化与年龄、性别、肿瘤分级无关。结论对一些肿瘤特异基因的甲基化状态进行联合检测可提高胶质瘤在血清学诊断水平上的灵敏度,为胶质瘤的筛查及早期诊断提供新的临床诊断思路。
[Abstract]:Objective to investigate the significance of combined detection of p16 and CDO1 gene methylation in plasma in the diagnosis of glioma. Methods the tumor tissues of 49 patients with glioma and their corresponding preoperative plasma levels were detected by methylation specific PCR(Methylation-specific polymerase chain reaction (PCR(Methylation-specific) in 10 patients with craniocerebral trauma and their corresponding preoperative plasma and 16 healthy controls. The methylation state of the p16 and cysteine dioxygenase 1 (CDO1) gene, To evaluate the clinical value of detection of p16 and CDO1 gene methylation in plasma for glioma. Results the methylation rates of p16 gene and CDO1 gene in glioma tissues were 53.1% and 42.9%, respectively, and the corresponding plasma methylation rates were 39.0% and 39.0%, respectively. The frequency of methylation of p16 and at least one CDO1 gene in tumor tissues and plasma was 75.5 / 37 / 49) and 67.3 / 49g / 49 respectively. The sensitivity of combined detection of p16 and CDO1 gene (methylation) in plasma was 67.3, which was significantly higher than that of single gene. No abnormal methylation of p16 and CDO1 genes was detected in 10 brain contusion tissues and 26 plasma samples in the control group, suggesting that the specificity of p16 and CDO1 (methylation) alone or in combination was 100%. Univariate analysis showed that plasma p16 and / or CDO1 gene methylation had no correlation with age, sex and tumor grade. Conclusion the combined detection of methylation status of some tumor-specific genes can improve the sensitivity of serological diagnosis of gliomas and provide a new clinical diagnosis method for the screening and early diagnosis of gliomas.
