双侧原发性乳腺癌与临床因素相关性分析

【摘要】:目的:就双侧原发性乳腺癌临床资料进行回顾性研究,探讨双侧原发性乳腺癌(BPBC)与临床相关因素的发病关系,为双侧原发性乳腺癌的发病相关因素研究提供一定的科学依据。方法:收集2011年1月~2015年12月新疆医科大学附属肿瘤医院确诊乳腺癌患者6700例,其中双侧原发性乳腺癌共105例,占同期乳腺癌患者的1.5%,sBPBC60例,mBPBC45例。同时随机抽取同期单侧乳腺癌患者105例作为对照组。选取患者均经我院病理确诊,有较完善的临床资料,统计并分析了包括发病年龄、家族史、病理类型、肿块大小、淋巴结转移情况、肿瘤分期、ER/PR、Her-2、Ki67等。采用卡方检验及非条件二元Logistic单因素和多因素回归方法逐步分析数据。结果:多因素Logistic回归分析结果显示,有家族史(OR=3.077,95%CI:1.244-7.609)、Her-2阳性(OR=2.283,95%CI:1.085-4.803)为双侧原发性乳腺癌的危险因素,年龄45岁(OR=0.451,95%CI:0.214-0.762)、ER/PR受体阳性(OR=0.417,95%CI:0.218-0.799)为其保护因素。结论:年龄45岁、ER/PR受体阳性为双侧原发性乳腺癌的保护因素,有家族史、Her-2阳性为危险因素。
[Abstract]:Objective: to study retrospectively the clinical data of bilateral primary breast cancer and to explore the relationship between bilateral primary breast cancer BPBCand clinical related factors, and to provide a scientific basis for the study of bilateral primary breast cancer. Methods: from January 2011 to December 2015, 6700 cases of breast cancer were diagnosed in the affiliated Cancer Hospital of Xinjiang Medical University, including 105 cases of bilateral primary breast cancer, which accounted for 1.5 sBPBC60 cases of mBPBC45 cases of breast cancer patients in the same period. At the same time, 105 patients with unilateral breast cancer were randomly selected as control group. All the patients were confirmed by pathology in our hospital and had perfect clinical data. The age of onset, family history, pathological type, tumor size, lymph node metastasis and tumor staging were analyzed. Chi-square test and non-conditional binary Logistic single factor and multi-factor regression were used to analyze the data step by step. Results: the results of multivariate Logistic regression analysis showed that there was a family history of 3.07795% CI: 1.244-7.609% Her-2 positive: 2.28395% CI: 1.085-4.803) as a risk factor for bilateral primary breast cancer. The age of 0.45195 CIW 0.214-0.762ERPR receptor positive OR 0.41795% CI0.218-0.799 was the protective factor. Conclusion: the age of 45 years old patients with ERR / PR receptor positive is the protective factor of bilateral primary breast cancer, and the family history is Her-2 positive as the risk factor.
