【摘要】：目的:系统评价含铂化疗方案治疗三阴性乳腺癌(TNBC)的临床疗效。方法:计算机检索The PubMed、Embase、CBM、CNKI、VIP、Conchrane Library等国内外重要的数据库。纳入含铂类与非铂类化疗药物治疗TNBC的随机对照试验,检索从建库至2016年5月相关资料,同时查阅所获取的参考文献索引,并由两位研究者对检索收集到的文献按照文章制定的纳入、排除标准进行独立筛选。采用RevMan5.3软件进行系统评价。结果:最终纳入10项研究,共TNBC患者1367例。Meta分析结果显示:两组在客观缓解率(ORR)、病理完全缓解率(p CR)、血小板减少及贫血方面差异有统计学意义(P0.05);在总生存率(OS)、无进展生存率(PFS)、恶心呕吐及白细胞减少方面差异无统计学意义(P0.05)。结论:含铂化疗方案可提高TNBC患者ORR、p CR,但患者血小板减少、贫血发生率高于非含铂化疗方案。临床选择过程中需权衡其利弊。
[Abstract]:Objective: to evaluate the clinical efficacy of chemotherapy regimen containing platinum in the treatment of triple negative breast cancer. Methods: The PubMedus Embase Library and other important databases at home and abroad were searched by computer. A randomized controlled trial of platinum-containing and non-platinum chemotherapeutic drugs for treatment of TNBC was conducted to retrieve relevant data from the establishment of the database to May 2016, as well as the index of references obtained, Two researchers independently sifted the collected literature according to the inclusion and exclusion criteria. RevMan5.3 software was used to evaluate the system. Results: eventually included in 10 studies, A meta-analysis of 1367 patients with TNBC showed that there were significant differences in objective remission rate (ORR), pathological complete remission rate (CR), thrombocytopenia and anemia between the two groups (P < 0.05), and in overall survival rate, progressive survival rate (P < 0.05), nausea and vomiting, and white. There was no significant difference in cell reduction (P 0.05). Conclusion: platinum-containing chemotherapy regimen can increase ORRP-CRs in patients with TNBC, but the incidence of anemia in patients with thrombocytopenia is higher than that in non-platinum chemotherapy regimen. In the course of clinical selection, the pros and cons should be weighed.
