miR-221与miR-222在甲状腺癌中的表达及临床意义

【摘要】:目的:甲状腺癌是最常见的内分泌系统的恶性肿瘤。在全球的发病率均呈不断上升的趋势,是增长速度最快的肿瘤之一。甲状腺结节的良恶性判定及早期手术治疗对患者取得良好预后具有关键性作用,因此寻找甲状腺癌诊断的分子标志物是目前研究的热点。近年来,多项研究发现miRNA与肿瘤的发生发展关系密切,并且mi RNA可通过多种靶基因调控甲状腺癌的发生和发展。当前的研究并不能完全解释miR-221与miR-222在甲状腺癌中的功能。本研究应用实时荧光定量PCR法检测甲状腺癌组织及甲状腺良性病变组织中mi R-221与miR-222的表达水平,分析其与临床病理指标之间关系,为研究甲状腺癌的发生发展、诊断及治疗提供新的思路。方法:1收集2014年7月-2016年7月河北医科大学第一医院甲状腺手术标本共65例。其中良性甲状腺组织标本38例,甲状腺癌组织标本27例。新鲜标本离体后迅速置于液氮中保存。所有患者术前未接受任何放疗及内分泌治疗。2采用实时荧光定量PCR法(quantitative real-time polymerase chain reaction,qRT-PCR)检测miR-221与miR-222在甲状腺癌组织及甲状腺良性病变组织中的表达。3应用SPSS 19.0软件对实验的结果数据进行统计分析。非正态分布的数据以P50(P25,P75)表示,对结果采用两独立样本的非参数检验,P0.05为差异有统计学意义。结果:1 miR-221在21例结节性甲状腺肿组织标本中的表达量为0.028(0.008,0.165),在17例甲状腺腺瘤组织中表达量为0.053(0.009,0.170),二者无明显差异(P0.05)。miR-222在21例结节性甲状腺肿组织标本中的表达量为0.003(0.002,0.045),在17例甲状腺腺瘤组织中表达量为0.008(0.002,0.041),二者无明显差异(P0.05)。2 mi R-221在27例癌组织标本中的表达量为3.155(0.137,55.796),高于38例甲状腺良性病变组织中的0.030(0.007,0.162),差异有统计学意义(P0.05)。miR-222在27例癌组织标本中的表达量为0.065(0.014,0.212),高于38例甲状腺良性病变组织中的0.006(0.002,0.038),差异有统计学意义(P0.05)。3 miR-221与mi R-222的表达与患者的性别、年龄、临床分期均无明显相关性(P0.05);且miR-221与淋巴结转移情况无明显相关性(P0.05),而miR-222与淋巴结转移情况相关(P0.05)。结论:1 miR-221与mi R-222在甲状腺癌组织中高表达,提示其与甲状腺癌的发生有关。2 miR-221与mi R-222与甲状腺良性病变的发生无明显相关。3 miR-222可能与甲状腺癌的进展密切相关。
[Abstract]:Objective: thyroid carcinoma is the most common endocrine system malignant tumor. In the world, the incidence of disease is on the rise, is one of the fastest-growing tumors. The diagnosis of benign and malignant thyroid nodules and early surgical treatment are crucial for the patients to obtain a good prognosis. Therefore, the search for molecular markers for thyroid cancer diagnosis is a hot topic at present. In recent years, many studies have found that miRNA is closely related to tumorigenesis and progression, and mi RNA can regulate the occurrence and development of thyroid carcinoma through a variety of target genes. Current studies do not fully explain the role of miR-221 and miR-222 in thyroid cancer. In this study, the expression of miR-221 and miR-222 in thyroid carcinoma and benign thyroid lesions was detected by real-time fluorescence quantitative PCR, and the relationship between miR-221 and clinicopathological indexes was analyzed in order to study the occurrence and development of thyroid carcinoma. Diagnosis and treatment provide new ideas. Methods from July 2014 to July 2016, 65 thyroid specimens were collected from the first Hospital of Hebei Medical University. There were 38 cases of benign thyroid tissue and 27 cases of thyroid carcinoma. Fresh specimens were quickly stored in liquid nitrogen in vitro. The expression of miR-221 and miR-222 in thyroid carcinoma and benign thyroid lesions was detected by real-time quantitative real-time polymerase chain reaction.qRT-PCR.2 the expression of miR-221 and miR-222 in thyroid carcinoma and benign thyroid lesions was detected by SPSS 19.0 software. A statistical analysis of the result data is carried out. The data of non-normal distribution were expressed as P50 / P25 / P75), and there was significant difference between the two independent samples by non-parametric test (P0.05). Results the expression of 1 miR-221 in 21 cases of nodular goiter was 0.028 0. 008 ~ 0. 165%, and in 17 cases of thyroid adenoma was 0. 053 ~ 0. 009 ~ 0. 170%. There was no significant difference between them in the expression of P0. 05 and MIR-222 in 21 cases of nodular goiter, and the expression was 0. 0030. 002. 045% in 21 cases of nodular goiter. In 17 cases of thyroid adenoma, the expression level was 0.008 / 0.002 / 0.041. There was no significant difference in the expression of P0.052.MIR-221 in 27 cases of carcinoma, which was higher than 0.0300.0.0070.162in 38 cases of benign thyroid lesions. The difference was statistically significant in 27 cases of carcinoma. The expression level in tissue samples was 0.065 ~ 0.014 ~ 0.212%, which was higher than that in 38 cases of benign thyroid lesions. The difference was statistically significant between the expression of P0.05.3 miR-221 and mi R-222 and the sex of the patients. There was no significant correlation between age and clinical stage (P 0.05), and there was no significant correlation between miR-221 and lymph node metastasis (P 0.05), but there was no significant correlation between miR-222 and lymph node metastasis (P 0.05). Conclusion the high expression of miR-221 and miR-222 in thyroid carcinoma suggests that there is no significant correlation between R- and miR-221 in thyroid carcinogenesis and the occurrence of benign thyroid lesions. 3 miR-222 may be closely related to the progression of thyroid carcinoma.
