食管癌患者血清中氧化肽测定及YB-1蛋白在食管癌中的表达

【摘要】:研究背景:食管癌的诊断主要依据患者的临床表现、影像检查及病理活检等,肿瘤标志物在食管癌的诊断及判定预后中特异性较差。有研究报道,氧化应激在多种肿瘤的发生发展中扮演了重要角色,而氧化应激相关参数,如总抗氧化状态(total antioxidant status,TAS),总氧化状态(total oxidant status,TOS)及氧化应激指数(oxidative stress index,OSI)等在患者血液中的表达量较健康人群均有不同程度的升高或降低。上述三个参数联合测定在乳腺癌及甲状腺癌中已有报道,且其表达量与疾病预后及分期相关,但在食管癌中联合测定的文章报道较少。有文献报道,在应激状态下,YB-1(Y-box-binding protein 1)蛋白的表达上调,但目前尚无文献报道氧化应激参数与YB-1蛋白的关系。目的:本研究通过测定食管癌患者血清中相关氧化应激参数与健康人群的表达水平、吸烟史、饮酒史、治疗效果及临床分期之间的关系,同时检测胃镜活检的食管癌标本中YB-1蛋白的表达水平与氧化应激参数之间的关系,为食管癌的临床诊断和判定预后提供理论依据。方法:随机收集我院92例不同临床分期及治疗前的食管癌患者和64例健康体检者的空腹血液,测定其血清中TOS及TAS含量,并计算出OSI值。比较食管癌患者与健康体检者血清中氧化应激参数的差异,同时还比较了氧化应激参数在食管癌不同临床分期、有无烟酒史、短期疗效及与YB-1蛋白表达水平之间的差异。统计学处理采用IBM SPSS 22.0统计软件进行统计分析,P0.05视为有统计学差异。结果:在食管癌患者中,TOS值较健康人群低,差异有统计学意义(P0.01),而食管癌患者的TAS值比健康人群明显低,差异统计学意义(P0.01),两组间OSI比值同样有统计学意义(P0.01)。TOS、TAS及OSI值与患者YB-1蛋白表达水平、临床分期、吸烟及饮酒史及短期疗效间无统计学意义(P0.05)。结论:(1)食管癌患者血清中TAS及TOS可能是潜在的生物标记物。(2)对ROC曲线分析发现,TOS、TAS对食管癌的诊断具有一定参考价值。(3)病检标本测定的YB-1蛋白的表达水平与食管癌患者血清中TOS、TAS及OSI进行对比无相关性。
[Abstract]:Background: the diagnosis of esophageal carcinoma is mainly based on the clinical manifestations, imaging examination and pathological biopsy. The specificity of tumor markers in the diagnosis and prognosis of esophageal cancer is poor. It has been reported that oxidative stress plays an important role in the occurrence and development of many kinds of tumors, and the parameters related to oxidative stress, The expression of total antioxidant (total antioxidant status tas, total oxidized (total oxidant status TOS and oxidative stress index (oxidative stress index OSI in the blood of the patients were higher or lower than those of the healthy subjects. The combined determination of these three parameters has been reported in breast cancer and thyroid carcinoma, and the expression of these parameters is related to the prognosis and stage of the disease, but there are few reports of combined determination in esophageal carcinoma. It has been reported that the expression of YB-1 (Y-box-binding protein 1) protein is up-regulated under stress, but the relationship between oxidative stress parameters and YB-1 protein has not been reported. Objective: to study the relationship between serum oxidative stress parameters and the expression level, smoking history, drinking history, therapeutic effect and clinical stage of patients with esophageal cancer. At the same time, the relationship between the expression of YB-1 protein and the parameters of oxidative stress was detected in the esophageal carcinoma specimens by gastroscopy biopsy, which provided a theoretical basis for the clinical diagnosis and prognosis of esophageal carcinoma. Methods: the fasting blood samples of 92 patients with esophageal cancer and 64 healthy controls were collected randomly. The serum TOS and TAS levels were measured and the OSI values were calculated. To compare the difference of serum oxidative stress parameters between esophageal cancer patients and healthy controls, and to compare the differences of oxidative stress parameters in different clinical stages of esophageal cancer, history of tobacco and alcohol, short-term efficacy and YB-1 protein expression level. Statistical analysis using IBM SPSS 22.0 statistical software was regarded as statistical difference (P0.05). Results: the values of TOS in patients with esophageal cancer were significantly lower than those in healthy subjects (P0.01), and the TAS values of patients with esophageal cancer were significantly lower than those of healthy subjects. There was no significant difference between the OSI ratio of the two groups (P0.01). TOSTAS and OSI values and YB-1 protein expression level, clinical stage, smoking and drinking history and short-term curative effect (P0.05). Conclusion: (1) TAS and TOS may be potential biomarkers in the serum of esophageal cancer patients. (2) the analysis of ROC curve shows that TOSTAS has some reference value in the diagnosis of esophageal carcinoma. (3) the expression level of YB-1 protein and the expression of YB-1 protein in the specimens of esophageal cancer and esophageal carcinoma are determined. There was no correlation between TOSTAS and OSI in serum.
