HCCR、Survivin和PTEN在人胃癌中的表达及临床意义

【摘要】:背景:胃癌(gastric cancer)是世界上最为常见的恶性肿瘤之一,胃癌(gastric cancer)的发生及发展是一个多阶段、多步骤、有序的过程,是多因素的综合结果,其中抑癌基因、癌基因及一些生长因子在其中发挥了重要的作用。目的:观察人宫颈癌基因(HCCR)、凋亡抑制蛋白(Survivin)、PTEN在胃癌组织、癌旁组织、正常胃组织中的表达特点,并探究它们与胃癌临床病理特征之间的关系。检测PTEN、Survivin和HCCR的表达及之间的关系,探讨HCCR、Survivin和PTEN在胃癌发生、发展、预后中的作用机理,探寻胃癌发生、发展的分子机理,了解其癌变的信号传导通路,并为胃癌的分子靶向治疗提供探索实验及临床治疗依据。方法:1、收集2012年11月-2013年11月期间甘肃省人民医院手术和内窥镜结合病理确诊的正常胃组织60例、癌旁组织60例、胃癌组织60例标本并经病理确诊。2、所选标本的患者在手术前均未接受放疗、化疗和各种针对性诊治,无并发其它肿瘤。3、应用免疫组化S-P法检测60例胃癌组织、60例癌旁组织、60例正常胃黏膜组织中HCCR和Survivin、PTEN基因蛋白的表达。所有实验数据均采用SPSS22.0统计软件包进行处理,统计学处理使用卡方检验和Spearman相关性分析,以P0.05为差异有统计学意义。结果:HCCR在60例正常胃组织中仅有2例阳性表达,阳性表达率为3.3%,在60例癌旁组织中27例呈阳性表达,其阳性表达率为45%;高于正常胃组织,在60例胃癌组织中43例为阳性表达,阳性表达率为71.7%;呈高表达,从正常胃组织、癌旁组织到胃癌组织,HCCR的阳性表达率呈逐渐递增趋势;在癌旁组织和胃癌组织中的表达明显高于正常组织,差异具有统计学意义(P0.05);在癌旁组织中的表达和胃癌组织中表达也具有统计学意义(P0.05)。PTEN在60例正常胃组织中有58例呈阳性表达,阳性表达率为96.7%,在60例癌旁组织中有47例呈阳性表达,阳性表达率为78.3%,在60例胃癌组织中有28例呈阳性表达,阳性表达率为46.7%。在正常胃组织、癌旁组织和胃癌组织中,PTEN的阳性表达率呈递减趋势,在癌旁组织和胃癌组织中明显低于正常胃组织中的表达,差异均具有统计学意义(P0.01)在癌旁组织和胃癌组织中的表达也具有统计学意义(P0.05)。Survivin在60例正常胃组织中有7例呈阳性表达,阳性表达率为11.7%,在60例癌旁组织中有26例呈阳性表达,阳性表达率为43.3%,在60例胃癌组织中有44例呈阳性表达,阳性表达率为73.3%。在正常胃组织、癌旁组织和胃癌组织中,Survivin的阳性表达率呈明显递增趋势,在癌旁组织和胃癌组织中明显高于正常胃组织中的表达,差异均具有统计学意义(P0.01)在癌旁组织和胃癌组织中的表达也具有统计学意义(P0.05)。HCCR和Survivin基因蛋白在胃癌组织中表达明显,PTEN蛋白在胃癌组织中低表达,三者差异有统计学意义(P0.05);Survivin基因蛋白表达与胃癌组织病理分级有关(P0.05)。结论:HCCR和Survivin基因蛋白在胃癌组织中均为高表达,为胃癌早期诊断提供了新的生物学指标,同时在胃癌的治疗方面,提出了新的治疗策略。PTEN基因在正常胃组织、癌旁组织、胃癌组织中表达呈递减趋势,表明PTEN基因与胃癌的发生密切相关,表明PTEN基因的表达可能成为临床评价胃癌恶性程度及其预后提供新的指标。
[Abstract]:Background: gastric cancer (gastric cancer) is one of the most common malignant tumors in the world. The occurrence and development of gastric cancer (gastric cancer) is a multistage, multistep and orderly process, which is a comprehensive result of multiple factors. The tumor suppressor gene, oncogene and some growth factors have played an important role in it. Objective: To observe the human cervical cancer base HCCR, apoptosis inhibitory protein (Survivin), PTEN expression in gastric cancer tissue, para cancer tissue and normal gastric tissue, and explore the relationship between them and the clinicopathological features of gastric cancer. The relationship between the expression of PTEN, Survivin and HCCR and the relationship between them are detected, and the mechanism of HCCR, Survivin and PTEN in the occurrence, development and prognosis of gastric cancer is explored. The molecular mechanism of the occurrence and development of gastric cancer, to understand the signal transduction pathway of its carcinogenesis, and to provide experimental and clinical basis for the molecular targeting therapy of gastric cancer. Methods: 1, 60 cases of normal gastric tissue, 60 cases of para cancer tissue, and gastric cancer group in Gansu people's hospital in November 2012, November, were collected by surgery and endoscopy combined with pathology. 60 specimens were weave and pathologically confirmed.2. The patients selected were not treated with radiotherapy, chemotherapy and various specific diagnosis and treatment and no other tumor.3 before operation. The immunohistochemical S-P method was used to detect 60 cases of gastric cancer tissue, 60 cases of para cancer tissue, and 60 normal gastric mucosa tissues, and the expression of HCCR and Survivin, PTEN gene protein. All of them were treated with SPSS22.0 software package. Statistical processing using chi square test and Spearman correlation analysis were statistically significant. Results: there were only 2 positive expressions in 60 normal gastric tissues in 60 cases, the positive expression rate was 3.3%, and 27 cases were positive in 60 para cancerous tissues, and the positive expression rate was 45%. The positive expression rate of 43 cases of normal gastric tissue was positive in 60 cases of gastric cancer, the positive expression rate was 71.7%, and the positive expression rate was gradually increasing from normal gastric tissue, para cancer tissue to gastric cancer tissue. The expression of HCCR was higher than normal tissue, and the difference was statistically significant (P0.05) in the para cancer tissue and gastric cancer tissue. Expression in tissue and expression in gastric cancer tissue were also statistically significant (P0.05).PTEN positive expression in 60 normal gastric tissues, positive expression rate was 96.7%, in 60 cases of para cancer tissues, 47 cases were positive, positive expression rate was 78.3%, 28 cases in 60 cases of gastric cancer were positive expression, the positive expression rate was 46.7%.. The positive expression rate of PTEN was decreasing in normal gastric tissue, para cancer tissue and gastric cancer tissue. The expression in para and gastric tissues was significantly lower than that in normal gastric tissue. The difference was statistically significant (P0.01) in the para cancer tissue and gastric cancer tissue (P0.05).Survivin in 60 normal gastric groups The positive expression rate of 7 cases was 11.7%. In 60 cases of para cancerous tissues, 26 cases were positive, the positive expression rate was 43.3%. In 60 cases of gastric cancer, 44 cases were positive, the positive expression rate was 73.3%. in normal gastric tissue, the positive expression rate of Survivin in the para cancer tissue and gastric cancer tissue was obviously increasing trend, in cancer. The expression of para tissue and gastric carcinoma was significantly higher than that in normal gastric tissue. The difference was statistically significant (P0.01) in the para cancer tissue and gastric cancer tissue (P0.05), the expression of.HCCR and Survivin gene protein in gastric cancer tissues was obvious, and the PTEN egg white was low in the gastric cancer tissue, and the difference between the three was statistically significant. Significance (P0.05); the expression of Survivin gene protein is related to the histopathological classification of gastric cancer (P0.05). Conclusion: the expression of HCCR and Survivin gene protein in gastric cancer tissues is high, which provides a new biological index for the early diagnosis of gastric cancer. At the same time, a new therapeutic strategy,.PTEN gene in normal gastric tissue and para cancer group, is proposed in the treatment of gastric cancer. The expression of the gastric cancer tissue is decreasing, which indicates that the PTEN gene is closely related to the occurrence of gastric cancer. It shows that the expression of PTEN gene may be a new index for the evaluation of the malignant degree and prognosis of gastric cancer.
