CD133在胃癌组织中的表达及其临床意义的系统评价

【摘要】:目的全面收集国内外发表的相关研究,系统评价CD133蛋白表达与胃癌及其临床病理学特征之间的关系,以了解CD133在胃癌发生、发展过程中的作用,以期为胃癌的治疗和预后干预提供循证医学依据。方法计算机检索中国生物医学文献数据库、中文科技期刊全文数据库、相关期刊论文、万方数据库、PubMed、EMbase、Web of Knowledge和The Cochrane Library,均从建库检索至2013年10月;并辅以手工检索,查找国内外公开发表的关于CD133蛋白表达与胃癌及其临床病理特征相关性的病例-对照研究。由两位研究者按照事先制定的纳入排除标准筛选文献、提取资料并评价方法学质量后,采用RevMan 5.2软件进行Meta分析。使用STATA 11.0进行Begg和Egger检验,并绘制Begg图和Egger图。最后,使用GRADEpro 3.6对各结果指标进行证据质量分级。结果最终纳入9个病例-对照研究,共623例患者。Meta分析结果显示:CD133蛋白在胃癌组与癌旁和正常胃组织组[OR=3.89,95%CI (1.87,8.11),P=0.0003]、淋巴结转移组与无淋巴结转移组[OR=2.75,95%CI (1.99,3.81),P0.00001]、胃癌Ⅲ~Ⅳ期组与Ⅰ-Ⅱ期组[OR=2.83,95%CI (2.13,3.76),P0.00001]、伴远处转移组与无远处转移组[OR=2.38,95%CI (1.47,3.85),P=0.0004]的表达差异均有统计学意义。在高中分化组与低分化组的表达差异无统计学意义[OR=1.70,95%CI (0.90,3.21),p=0.10],但敏感性分析结果显示该指标Meta分析结果不稳定,其他结果稳定。发表偏倚检测结果显示本系统评价不存在发表偏倚。证据质量分级结果显示3个结果指标为中等质量,2个指标为低质量。结论CD133蛋白在胃癌组织中的表达比癌旁组织及正常胃组织高,而且其表达与淋巴结转移、远处转移及临床分期等明显相关,说明CD133可能在胃癌发生、发展过程中起重要作用。上述结论尚需更多大样本、设计严谨的高质量研究加以验证。
[Abstract]:Objective to investigate the relationship between CD133 protein expression and gastric cancer and its clinicopathological features in order to understand the role of CD133 in the carcinogenesis and development of gastric cancer. In order to provide evidence-based medicine basis for the treatment and prognosis intervention of gastric cancer. Methods Chinese biomedical literature database, Chinese scientific and technological journal full-text database, Chinese journal full-text database, Wanfang database were searched by computer from the construction of the database to October 2013, and the database was retrieved by hand. A case-control study on the correlation between CD133 protein expression and gastric cancer and its clinicopathological features was studied.
