KISS-1和PPARγ在食管鳞癌组织中的表达及其临床意义

【摘要】:目的:食管鳞癌是人类最常见的消化道恶性肿瘤之一,确切的发病机制至今尚不明确,普遍认为与生活习惯、饮酒、吸烟、遗传以及环境社会等因素有关。研究发现Kiss-1和PPARγ基因在人类多种恶性肿瘤发生发展过程中发挥了重要作用。目前,关于Kiss-1和PPARγ与食管鳞癌之间的关系的研究尚无较多报道。本研究选择Kiss-1和PPARγ两种基因作为研究内容,利用实时定量荧光PCR法检测两种基因在食管鳞癌组织及正常食管黏膜组织中的表达情况,并分析两种基因与食管鳞癌患者临床病理资料之间的关系,借以探讨其在食管鳞癌发生中的作用及临床意义。方法:1收集2012年1月~2015年12月,河北医科大学第一医院、河北医科大学第四医院手术切除的食管鳞癌组织标本40例(患者术前均未接受任何形式的化疗和放疗)。每例标本分别取自食管鳞癌原发肿瘤组织及切缘正常黏膜(正常黏膜组织由术后病理证实,均无癌组织侵犯)。肿瘤组织切除后迅速置于液氮中短暂保存,随后置于-80℃超低温冷冻储存箱保存。2利用实时定量荧光PCR方法检测40例食管鳞癌及食管正常黏膜组织中Kiss-1与PPARγ的表达水平,比较两种基因在组间表达的差异,分析Kiss-1与PPARγ的表达与食管鳞癌患者临床病理特征的关系。3采用SPSS19.0及GraphPad Prism 5.0统计软件进行数据的计算及统计分析,非正态分布的数据以中位数(四分位数)表示,两组间配对样本采用非参数检验,以P0.05表示差异存在有统计学意义。结果:1 Kiss-1在40例食管鳞癌组织中表达水平为0.0256(0.0090,0.0965),正常食管黏膜中表达水平为0.0781(0.0424,0.3108),说明Kiss-1在食管鳞癌组织中的表达水平明显低于正常食管黏膜(P0.05),差异具有统计学意义;2 Kiss-1在有淋巴结转移的食管鳞癌组织中表达水平为0.0110(0.0056,0.0300),明显低于无淋巴结转移的食管鳞癌组织中的表达水平0.0591(0.0230,0.2405)(P0.01);Kiss-1的表达与其他病理学资料无明显关系,差异无统计学意义(P0.05);3 PPARγ在40例食管鳞癌组织中表达水平为0.2624(0.0677,0.5715),正常黏膜组织中表达水平为0.5280(0.1225 1.1839),说明PPARγ在食管鳞癌组织中的表达水平明显低于正常食管黏膜(P0.05),差异具有统计学意义;4 PPARγ在有淋巴结转移的食管鳞癌组织中表达水平为0.1797(0.0570,0.3668)明显低于无淋巴结转移的食管鳞癌组织中的表达水平为0.4333(0.0743,0.8477)(P0.05);PPARγ在低分化的食管鳞癌组织中表达水平为0.0535(0.0239,0.0787)明显低于中高分化的食管鳞癌组织中的表达水平为0.4076(0.1866,0.7157)(P0.01);PPARγ的表达与其他病理学资料无明显关系,差异无统计学意义(P0.05)。结论:1 Kiss-1和PPARγ在食管鳞癌组织中均呈现低表达,提示其在食管鳞癌的发生、发展中可能具有抑癌作用。2 Kiss-1在有淋巴结转移组中表达水平明显低于无淋巴结转移组,提示Kiss-1在食管鳞癌发展过程中可能起到抑制肿瘤转移的作用。3 PPARγ在有淋巴结转移组中表达水平明显低于无淋巴结转移组,在低分化食管鳞癌组织中表达水平明显低于中高分化组,提示PPARγ的表达水平对食管鳞癌预后判断具有一定的临床价值。
[Abstract]:Objective: esophageal squamous cell carcinoma is one of the most common malignant tumor of the digestive tract in human. The exact pathogenesis is still unclear. It is generally believed that it is related to living habits, drinking, smoking, heredity and environmental society. The study found that Kiss-1 and PPAR gamma play an important role in the development of various human malignant tumors. The study of the relationship between Kiss-1 and PPAR gamma and esophageal squamous cell carcinoma has not been reported. This study selected two genes of Kiss-1 and PPAR gamma as research contents. The expression of two genes in esophageal squamous cell carcinoma and normal esophageal mucosa was detected by real-time quantitative fluorescent PCR, and two genes were analyzed with esophageal squamous cell carcinoma. The relationship between clinicopathological data is to explore its role and clinical significance in the occurrence of squamous cell carcinoma of the esophagus. Methods: 1 in January 2012, December ~2015, the first hospital of Hebei Medical University, and 40 cases of esophageal squamous cell carcinoma excised by the fourth hospital of Hebei Medical University (no form of chemotherapy and radiotherapy were accepted before the operation). Each specimen was taken from the primary tumor tissue of the squamous cell carcinoma of the esophagus and the normal mucous membrane of the cutting edge (normal mucosa was confirmed by postoperative pathology, all of which had no cancer tissue).
