TACE联合氩氦刀冷冻治疗对肝癌血清血管内皮生长因子的影响

【摘要】:目的对原发性肝癌患者分别采用TACE (transcatheter arterial chemoemboli zat ion, TACE)治疗和TACE联合氩氩刀冷冻消融(argon-helium cryoablation therapy, AHC)治疗,分别检测患者治疗前后血清中血管内皮生长因子(vascular endothelial growth factor, VEGF)水平,研究2种治疗方法对肝癌患者血清中血管内皮生长因子(VEGF)表达水平影响。方法 收集湖南省人民医院介入血管外科从2013年3月至2014年3月期间50例原发性肝癌患者,分别行TACE治疗(对照组,n=25)和氩氦刀联合TACE治疗组(实验组,n=25)。分别在治疗前的第1d,治疗后的第1d、第3d、第7d、第28d,抽取静脉血测定VEGF表达水平。结果 (1)TACE治疗组血清VEGF水平的检测:TACE组(对照组)术前为286.04pg/ml,术后第1d、3d、7d、28d的血清VEGF值分别为547.04、411.04、313.87、301.38pg/ml。TACE术后肝癌患者的血清VEGF表达水平呈现先升高后降低的动态变化趋势,术后第1d VEGF表达水平最高,与术前比较差异有统计学意义(P0.05) 术后第3、7d肝癌患者血清中VEGF水平较术后1d降低,与术前比较,差异有统计学意义(P0.05);术后第28d较术前VEGF表达水平仍稍高,但差异无统计学意义(P0.05)。(2) TACE联合氩氦刀治疗组(实验组)术前为283.69pg/ml,术后第1d、3d、7d、28d的血清VEGF值分别为509.42、380.01、194.64、279.85pg/ml,联合治疗后肝癌患者的VEGF表达水平呈现先升高后降低再升高的动态变化趋势,术后第1、3d VEGF表达水平升高,与术前比较差异有统计学意义(P0.05) 术后第7d肝癌患者血清中VEGF水平较术前降低明显,差异有统计学意义(P0.05);术后第28d VEGF表达水平与术前比较差异无统计学意义(P0.05)。(3)联合治疗组术后各个时间点与TACE治疗组比较,血清中VEGF的水平均较低,且差异均有统计学意义(P0.05),术后7d联合组较TACE组有明显下降趋势。结论(1) TACE治疗原发性肝癌,患者血清中VEGF表达水平升高;(2) TACE联合氩氦刀冷冻治疗原发性肝癌后,患者血清中VEGF的表达水平比TACE治疗要低;(3) TACE联合氩氦刀冷冻治疗要优于单纯TACE治疗。
[Abstract]:Objective to detect the level of vascular endothelial growth factor (vascular endothelial growth factor, VEGF) in serum of patients with primary liver cancer before and after treatment with TACE (transcatheter arterial chemoemboli zat ion, TACE) and TACE combined with argon ablation (argon-helium cryoablation therapy, AHC). To study the effect of two treatment methods on the expression of vascular endothelial growth factor (VEGF) in serum of patients with liver cancer. Methods from March 2013 to March 2014, 50 patients with primary liver cancer in the interventional vascular surgery of Hunan Provincial people's Hospital were treated with TACE (control group, nong25) and argon-helium combined with TACE treatment group (experimental group, nong25). The expression of VEGF was measured on the first day before treatment, on the first day, on the third day, on the seventh day and on the 28th day after treatment. Results (1) the level of serum VEGF in TACE group was 286.04pg / ml before operation in TACE group (control group), and the serum VEGF level of TACE group (control group) was increased first and then decreased after 547.04411.04313.87301.38pg/ml.TACE operation. The expression of VEGF was the highest on the 1st day after operation, which was significantly different from that before operation (P0.05). The level of serum VEGF in patients with liver cancer on the 3rd day after operation was lower than that on the 1st day after operation (P0.05). On the 28th day after operation, the expression of VEGF was still slightly higher than that before operation. However, there was no significant difference (P0.05). (2) between TACE and argon-helium therapy group (experimental group) was 283.69 PG / ml before operation, and the serum VEGF value on the 1st day after operation was 509.42380.01 194.279.85 pg-1 路ml, respectively. After combined therapy, the VEGF expression level of HCC patients increased first and then decreased, and then increased. The expression of VEGF increased on the 1st day after operation, which was significantly higher than that before operation (P0.05). The level of serum VEGF in patients with liver cancer on the 7th day after operation was significantly lower than that before operation (P0.05). There was no significant difference in the expression of VEGF between the two groups on the 28th day after operation (P0.05). (3). Compared with the TACE group, the serum VEGF level in the combined treatment group was lower than that in the TACE group at all time points after operation (P0.05). (3). And the difference was statistically significant (P0.05), 7 days after the combined group than the TACE group has a significant downward trend. Conclusion (1) the expression of VEGF in serum of patients with primary liver cancer treated with TACE is higher than that of patients with primary liver cancer, (2) the expression of VEGF in serum of patients with primary liver cancer treated with TACE combined with cryotherapy with argon-helium knife is lower than that with TACE. (3) TACE combined with argon-helium cryotherapy is superior to TACE alone.
