Ras相关区域家族10在胃癌中的表达及与胃癌患者预后相关的临床病理学研究

【摘要】:背景:Ras相关区域家族10(RASSF10)已经被发现在多种恶性肿瘤中作为抑癌基因而异常表达,并且表达异常的RASSF10与肿瘤的恶性程度及其患者的临床转归及预后密切相关。在胃癌中,RASSF10的表达情况和作用机制尚不清楚。目的:本研究旨在探讨RASSF10基因在人类胃癌中的表达及与胃癌患者预后相关的临床病理学参数的关系,为胃癌的临床诊治提供客观依据。方法:选取2008年1月至2010年12月在第二军医大学附属长征医院胃癌手术标本共105例。所有患者临床随访时间至少五年或随访至死亡。另取我院2015年3月至2015年6月胃癌组织标本及相对应癌旁组织标本各15例,于手术切除后切取一定大小立刻在液氮中进行速冻,并保持在-80℃冰箱中行Western blot和q RT-PCR分析。应用Western blot、q RT-PCR、免疫组织化学等方法检测RASSF10在胃癌组织和相应癌旁正常组织中的表达情况,并分析其在胃癌不同TNM分期中的表达特点,及其与临床病理学参数和预后的关系。在此基础上,应用Western blot、q RT-PCR的方法检测多株胃癌细胞株和永生化胃上皮细胞GES1细胞中RASSF10的表达情况。利用去甲基化试剂5-杂氮-2'-脱氧胞苷(5-aza-2'-deoxycytidine,5-Aza-d C)处理所选取的与GES1细胞相比表达差异最明显的BGC-823细胞系,通过细胞划痕实验来检测细胞的迁移能力,通过平板克隆形成实验来检测胃癌细胞的增殖能力和活力。最后,运用SPSS 21.0统计软件进行统计学分析,计量资料采用t检验,计数资料采用χ~2检验,以P0.05为差异有统计学意义。结果:在15例冰冻胃癌组织和相对应癌旁组织中,qRT-PCR结果显示RASSF10在胃癌组织中的表达与相匹配的癌旁正常组织相比显著下降。Western blot实验结果进一步证实了qRT-PCR的结果,RASSF10在胃癌组织中的表达水平的确低于相配对的癌旁正常组织。并且随着临床TNM分期的进展,Ⅲ、Ⅳ期胃癌患者中该基因表达水平与Ⅰ、Ⅱ期胃癌患者相比,呈现逐渐下降趋势。免疫组织化学染色结果提示在所选取的105例胃癌组织中RASSF10阳性表达率为38.1%(40/105),阴性表达率为61.9%(65/105),在所选取的癌旁正常组织中RASSF10的阳性表达率为61.9%(65/105),阴性表达率为38.1%(40/105),两者差别具有统计学意义(p0.05)。进一步统计学分析显示,RASSF10表达水平与肿瘤大小、肿瘤分型、分化程度、浸润深度、淋巴结转移程度有关,差异均有统计学意义(所有p0.05);RASSF10的表达水平与患者年龄、性别、肿瘤部位等差别无明显统计学意义(所有p0.05)。通过Kaplan-Meier生存分析显示RASSF10表达阴性患者有着更差的预后(p=0.008)。Cox比例风险回归模型分析显示:RASSF10表达(HR=0.528,95%CI:0.286-0.975,p=0.041)与AJCC临床分期(HR=4.994,95%CI:1.998-12.486,p=0.001)可作为判断胃癌预后的独立的预后因子。体外细胞实验结果显示,RASSF10在多株胃癌细胞系中的表达水平均低于正常胃上皮细胞GES1细胞。5-Aza-d C处理BGC-823细胞后,RASSF10的表达水平升高。细胞平板克隆实验结果提示,去甲基化试剂处理组细胞的克隆数较未处理组克隆数明显减少。细胞划痕实验结果提示处理组的细胞从周边迁移到中央的细胞数明显减少,迁移距离明显缩短。结论:RASSF10蛋白在胃癌组织中较相应癌旁组织表达明显下调,并且随着临床分期及病理分期的进展,RASSF10的表达水平下调更加明显。统计学分析显示,其表达水平的高低与胃癌临床病理学参数密切相关,且其在胃癌中表达下降可预测不良预后。这提示我们RASSF10或许可以作为胃癌诊断与预后的潜在生物标记物。在体外机制研究中,我们发现RASSF10在胃癌细胞系中的表达水平较GES1细胞表达下降,BGC-823细胞系中下降更加明显。去甲基化试剂处理后,RASSF10表达增多,并可明显减少胃癌细胞的克隆形成数以及向划痕中央迁移的距离,降低胃癌细胞的增殖能力和活力,并能够显著降低胃癌细胞的迁移能力。
