中期因子在结直肠癌患者外周血中的表达及其临床意义

【摘要】:目的:探索结直肠癌(Colorectal cancer,CRC)患者外周血中的中期因子(Midkine,MK)在诊断疾病和对结直肠癌预后判断方面的价值。方法:以2014年1月到12月就诊于山西省肿瘤医院结直肠肛门外科的126例结直肠癌患者为研究组,以山西省肿瘤医院体检中心75例健康体检者为对照组,分别检测两组外周血中MK蛋白以及癌胚抗原(Carcino-embryonic antigen,CEA)的含量。两组样本分别采用成组t检验进行比较,多组样本采用方差分析方法及LSD-t检验方法进行比较。结果:结直肠癌实验组较健康体检对照组外周血MK蛋白含量明显增高(p0.01)。I+II期、III期、IV期结直肠癌患者外周血中MK蛋白含量依次增高,分别为431±88、498±71、600±78 pg/ml,对三者进行两两比较,差异均有统计学意义(p0.01)。在诊断结直肠癌方面,外周血MK蛋白的诊断价值要高于外周血CEA,通过计算二者ROC曲线下方面积分别为0.878和0.824,其差异有统计学意义(p0.05)。结论:(1)外周血MK蛋白在诊断结直肠癌方面要优于传统肿瘤标记物CEA。(2)MK的升高也预示着结直肠癌患者较差的预后,表明外周血MK可能成为诊治结直肠癌的有效生物学标记物。
[Abstract]:Objective: to explore the value of metaphase factor (Midkine,MK) in peripheral blood of patients with colorectal cancer (Colorectal cancer,CRC) in diagnosis and prognosis of colorectal cancer. Methods: from January to December, 2014, 126 patients with colorectal and anal surgery in Shanxi Cancer Hospital were selected as study group and 75 healthy persons in Shanxi Cancer Hospital as control group. The contents of MK protein and carcinoembryonic antigen (Carcino-embryonic antigen,CEA) in peripheral blood were measured. Two groups of samples were compared by group t test, and many groups were compared by ANOVA and LSD-t test. Results: the content of MK protein in peripheral blood of colorectal cancer group was significantly higher than that of healthy control group (p0.01). I II phase, III phase, IV phase, MK protein content was 431 卤88498 卤71600 卤78 pg/ml, respectively). The differences were statistically significant (p 0.01). In the diagnosis of colorectal cancer, the diagnostic value of peripheral blood MK protein was higher than that of peripheral blood CEA,. The area under the ROC curve was 0.878 and 0.824, respectively, and the difference was statistically significant (p0.05). Conclusion: (1) the increase of MK protein in peripheral blood is better than that of CEA. (2) MK in the diagnosis of colorectal cancer. The results suggest that peripheral blood MK may be an effective biological marker for the diagnosis and treatment of colorectal cancer.
