维生素D受体在胃癌组织中的表达及临床意义

【摘要】:背景及目的近年来大量的细胞、动物学实验研究结果表明维生素D具有广泛的抗肿瘤作用,然而流行病学调查结果并不支持维生素D是胃癌的保护性因素这一说法,目前认为其生物学功能包括抗肿瘤作用须由维生素D受体(VDR)介导,于此我们大胆猜测胃癌组织中VDR可能异常表达(低表达),本研究旨在检测胃癌组织、癌旁正常胃粘膜组织以及癌前病变胃粘膜组织中VDR的表达情况并分析其临床意义。材料及方法采用免疫组织化学法检测不同类型胃粘膜组织中VDR的表达情况,卡方检验法检验其表达差异有无统计学意义,并分析其与胃癌患者临床各项参数间的关系。胃癌组织及癌旁正常胃粘膜组织来源于甘肃省人民医院92例行胃癌根治术患者的手术标本,癌前病变组织包括慢性萎缩性胃炎(50例)、肠上皮化生(52例)、不典型增生(46例)均收集于甘肃省人民医院内镜中心。结果胃粘膜组织中VDR主要表达于细胞质,胞核基本无表达。相较于癌旁正常胃粘膜(82.61%)及癌前病变胃粘膜组织(73.64%),VDR在胃癌组织中呈低表达状态(57.61%),具有明显的统计学差异(P=0.001)。在胃癌组织中,高、中、低分化组间VDR表达量依次降低,差异具有统计学意义(P=0.016);相较于小瘤体组(5 cm),其在大瘤体组(≥5 cm)中低表达,同样具有统计学差异(P=0.009)。比较不同类型胃粘膜组织中VDR阳性表达率,结果显示随着胃癌的发生发展,其表达呈一下降趋势。结论胃粘膜组织细胞中VDR主要定位于胞质,提示维生素D类药物对胃癌细胞的作用可能限于非基因途径或其基因途径较复杂;VDR在胃癌组织尤其是低分化胃癌组织中呈低表达状态;瘤体越大的胃癌组织VDR含量越低;VDR低表达并不仅仅发生在胃癌形成后,其异常表达始于癌前病变。VDR可能会成为判断胃癌预后的一项指标。
[Abstract]:Background and objective in recent years, a large number of cells, animal experimental results show that vitamin D has a wide range of anti-tumor effects, but epidemiological findings do not support the claim that vitamin D is a protective factor for gastric cancer. It is believed that its biological function including antitumor action should be mediated by vitamin D receptor (VDR). In this study, we venture to speculate that there may be abnormal expression (low expression) of VDR in gastric cancer tissues. Expression of VDR in adjacent normal gastric mucosa and precancerous gastric mucosa and its clinical significance were analyzed. Materials and methods Immunohistochemical method was used to detect the expression of VDR in different types of gastric mucosal tissues, and chi-square test was used to detect the difference between the expression of VDR and the clinical parameters of patients with gastric cancer. Gastric cancer tissues and adjacent normal gastric mucosa tissues were collected from 92 patients undergoing radical gastrectomy in Gansu Provincial people's Hospital. The precancerous lesions included chronic atrophic gastritis (50 cases) and intestinal metaplasia (52 cases). Atypical hyperplasia (46 cases) was collected in endoscopic center of Gansu Provincial people's Hospital. Results VDR was mainly expressed in the cytoplasm of gastric mucosa, but not in the nucleus. Compared with normal gastric mucosa (82.61%) and precancerous gastric mucosa (73.64%), the expression of), VDR in gastric cancer tissues was lower (57.61%), with significant statistical difference (P0. 001).
