胃泌素释放肽前体联合神经元特异性烯醇化酶对小细胞肺癌化疗疗效的评价

【摘要】:目的:化疗是治疗肺癌的关键手段之一,对化疗疗效的评价非常重要,Pro GRP和NSE在SCLC中都有不同程度表达,本实验旨在研究SCLC患者化疗前后血清Pro GRP及NSE水平变化,探讨Pro GRP和NSE是否有望成为预测SCLC化疗疗效的可靠指标。方法:健康对照组来自弋矶山医院体检中心,SCLC患者经病理学组织确诊,存在可评价病灶,符合化疗适应证。SCLC患者化疗前1天及化疗2、4周期后20天和健康对照组抽取静脉血液3 m L,离心取上层血清于零下80℃下冻存。使用ELISA方法测定血清Pro GRP和NSE水平在化疗前后的变化。结果:一、(1)与对照组比较,SCLC患者化疗前与化疗2周期后Pro GRP、NSE浓度明显高于正常对照组(P≤0.05);化疗4周期后与对照组比较,P0.05。(2)化疗前后对比,化疗前分别与化疗2、4周期后Pro GRP,NSE浓度比较,P≤0.05;化疗2、4周期后相比,P0.05。二、(1)各组中化疗前后Pro GRP比较,CR(完全缓解)、SD(稳定)、PD(进展)组显示化疗前和化疗2、4周期后两两比较,P≤0.05;PR(部分缓解)组,化疗前分别与化疗2、4周期后相比,P≤0.05;化疗2、4周期后相比,P0.05。(2)各组中化疗前后NSE比较,CR、PR、SD组显示化疗前分别与2、4周期后比较,P≤0.05;化疗2、4周期后相比,P0.05。PD组化疗前和化疗2周期后比较,化疗2、4周期后相比,P≤0.05;化疗前和化疗4周期后相比,P0.05。三、(1)化疗前4组比较,CR、PR、SD、PD组Pro GRP、NSE浓度比较,P0.05。(2)化疗2周期后4组比较,CR组与PR、SD、PD组Pro GRP比较,P≤0.05;PR、SD、PD组Pro GRP比较,P0.05。CR与SD、PD组NSE比较,PR与PD组NSE比较,P≤0.05;PR与CR、SD组NSE比较,SD、PD组NSE比较,P0.05。(3)化疗4周期后,CR、PR、SD组Pro GRP比较,P0.05;PD组与CR、PR、SD组比较,P≤0.05。NSE比较与化疗2周期后NSE化疗前后比较相似,除SD、PD组比较,P≤0.05。结论:1.化疗前Pro GRP、NSE浓度明显高于健康人群,化疗后Pro GRP、NSE的浓度均有不同程度下降,以化疗2周期后下降明显。2.SCLC患者化疗2、4周期后Pro GRP、NSE浓度比较,P0.05,但Pro GRP、NSE水平仍呈小幅下降趋势。3.化疗后CR、PR、SD、PD组血清Pro GRP、NSE浓度下降幅度不一,CR组下降明显,Pro GRP对化疗疗效的监测更有意义。化疗前后血清Pro GRP、NSE浓度之间呈正相关。4.Pro GRP、NSE联合检测SCLC与单独检测的敏感性差异不大,特异性较单独检测高。

According to the indication of chemotherapy, venous blood was taken from SCLC patients 1 day before chemotherapy, 20 days after 2 cycles of chemotherapy and healthy control group.
