二维超声征象预测PTC风险的分子病理学研究

【摘要】:目的:通过结合二维超声图像特征与长链非编码RNA HOTAIR在甲状腺乳头状癌中的分子病理学研究,探讨二维超声图像特征术前评估甲状腺乳头状癌风险的价值。方法:1.从TCGA数据库下载HOTAIR基因表达谱及临床参数信息,分析HOTAIR在甲状腺乳头状癌中的表达规律及其与临床病理参数的相关性。独立样本t检验比较两组样本中HOTAIR的表达水平;卡方检验检测HOTAIR表达水平与临床病理参数的相关性;Kaplan-Meier曲线对比HOTAIR高表达组与HOTAIR低表达组患者的生存时间。2.回顾性归纳总结34例甲状腺乳头状癌病灶的二维超声图像特征,包括数目、大小、边缘、纵横比、内部回声、钙化。3.应用RT-q PCR检测34例甲状腺乳头状癌及10例甲状腺正常组织中HOTAIR的表达水平,独立样本t检验比较两组样本中HOTAIR的表达水平。4.卡方检验或Fisher确切概率法分析各二维超声图像特征与HOTAIR表达规律的相关性。结果:TCGA数据库统计结果表明甲状腺乳头状癌组织中HOTAIR表达水平较甲状腺正常组织明显升高(P0.05),HOTAIR高表达与肿瘤大小(P=0.032)、淋巴结转移(P=0.009)、TNM分期(P=0.038)相关;HOTAIR高表达的甲状腺乳头状癌患者总生存时间明显短于低表达的患者(P=0.03);所纳入病例病灶的二维超声图像多表现为边缘不规则(91.2%)、纵横比≥1(76.5%)、实性低回声(94.1%)、伴微钙化(67.6%);RT-q PCR方法检测的纳入病例样本中HOTAIR表达水平与TCGA数据库统计结果一致,同样表现为在甲状腺乳头状癌组织中表达上调(P0.05),其中纵横比≥1与HOTAIR高表达有相关。结论:HOTAIR可能参与了甲状腺乳头状癌的发生发展过程,是甲状腺乳头状癌的不良预后因素;甲状腺乳头状癌图像特征中纵横比≥1与长链非编码RNA HOTAIR的高表达相关,是甲状腺乳头状癌术前风险评估的危险因素,可为其个体化治疗提供依据。
[Abstract]:Aim: to evaluate the risk of thyroid papillary carcinoma by combining the characteristics of two-dimensional ultrasound and long-chain non-coding RNA HOTAIR in the study of molecular pathology of papillary thyroid carcinoma. Methods: 1. HOTAIR gene expression profile and clinical parameter information were downloaded from TCGA database to analyze the expression pattern of HOTAIR in papillary thyroid carcinoma and its correlation with clinical pathological parameters. Independent sample t test was used to compare the expression level of HOTAIR in two groups; Chi square test was used to detect the correlation between HOTAIR expression level and clinicopathological parameters; Kaplan-Meier curve was used to compare the survival time of patients with high expression of HOTAIR and low expression of HOTAIR. The ultrasonographic features of 34 cases of papillary thyroid carcinoma were retrospectively summarized, including number, size, margin, aspect ratio, internal echo, calcification. RT-q PCR was used to detect the expression of HOTAIR in 34 cases of papillary thyroid carcinoma and 10 cases of normal thyroid tissue. T test of independent sample was used to compare the expression level of HOTAIR in the two groups. Chi-square test or Fisher exact probability method was used to analyze the correlation between the characteristics of each two-dimensional ultrasound image and the expression of HOTAIR. Results: the results of TCGA database showed that the expression level of HOTAIR in papillary thyroid carcinoma was significantly higher than that in normal thyroid tissues (P0.05), the expression of), HOTAIR was higher than that in normal thyroid tissues (P0. 032), lymph node metastasis (P < 0. 009), and tumor size (P < 0. 032). TNM stage (P0. 038) was correlated; The total survival time of thyroid papillary carcinoma patients with high expression of HOTAIR was significantly shorter than that of patients with low expression (P < 0. 03).
