小细胞肺癌保护海马回区脑预防照射的探索性临床研究

【摘要】:[目的]比较三种放疗技术保护海马回区的剂量学与毒副反应的相关性研究。[方法]从2014年4月至2015年3月,9例局限期小细胞肺癌患者入组。脑部核磁共振检查排除脑转移,应用Tomotherapy、IMRT 及 VMAT技术,对海马回区进行保护照射,放疗的处方剂量为95%PTV 25Gy/2.5Gy/10Fx。初步分析剂量分布,治疗计划优劣。根据CTC AE4.0, MMSE量表评价毒副反应及认知功能,并分析脑转移率。[结果]9例患者均应用Tomotherapy、IMRT、VMAT进行计划比较和剂量学研究。Tomotherapy计划可将海马回的平均剂量降至5.76Gy,外扩减量区平均剂量降至6.68Gy,较处方剂量分别降低77%和73%,海马回最大剂量7.14Gy,平均处方剂量覆盖为94.56%。IMRT 16.99Gy、18.26Gy,较处方剂量分别降低32%和27%。VMAT计划的海马回及外扩区平均剂量分别为17.81Gy、19.05Gy,较处方剂量分别降低29%和24%。9例患者中1例死亡,2例出现多发脑转移,未出现海马回区的转移。主要的毒副作用为轻度头疼、头晕。复诊MMSE(简易智能状态评价,满分30分,大于24为正常)平均得分29.3分,疗后6月复查记忆力平均得分5.86分(满分6分),较疗前无明显差异。MMSE评分未见明显下降,认知及记忆功能无明显受损。[结论]Tomotherapy技术保护海马回区的脑预防照射具有明显的剂量学优势,无明显毒副作用,患者疗后6个月未出现明显认知功能障碍。
