【摘要】:目的:观察肾虚型绝经后骨质疏松症患者骨组织中组蛋白去甲基化酶JMJD2A、 JMJD2B和雌激素受体ERα.ERβ的基因及蛋白表达水平变化,并分析组蛋白去甲基化酶JMJD2A、JMJD2B与骨密度的相关性以及JMJD2A、JMJD2B与ERα、ERβ的相关性,以期发现组蛋白去甲基化酶JMJD2与肾虚型绝经后骨质疏松症的联系,为治疗绝经后骨质疏松症寻找新的靶点。方法:选择于2014年10月至2015年6月广州中医药大学第一附属医院骨科收治的因髋关节骨性关节炎行全髋关节置换术的绝经后女性患者30例,年龄在50岁至70岁,术前使用双能X线骨密度仪测量患者术侧股骨颈骨密度,分为对照组15例(即骨量正常组,股骨颈BMD T值-1.0)和实验组15例(即绝经后骨质疏松组,股骨颈BMD T值≤-2.5),术中取术侧股骨颈处松质骨备用。应用荧光实时定量PCR和Western blot法分别检测松质骨骨组织中JMJD2A、JMJD2B、ERα和ER β基因及蛋白的表达水平。实验数据使用SPSS20.0软件处理,P0.05认定为有统计学意义。采用Pearson相关性分析法分析JMJD2A、JMJD2B基因及蛋白表达与骨密度的相关性,分析JMJD2A、JMJD2B 与 ER α、ERβ基因及蛋白表达之间的相关性。结果:1、与骨量正常组相比,绝经后骨质疏松组骨组织中JMJD2A、JMJD2B、 ERα及ER β基因及蛋白表达水平均明显下降,组间差异具有统计学意义(P值均0.01)。与骨量正常组相比,绝经后骨质疏松组骨组织中OPG mRNA表达水平下降,RANKL及RANKL/OPG mRNA表达水平均升高,两组间差异具有统计学意义(P值均0.01)。2、相关性分析显示绝经后女性股骨颈骨密度与JMJD2A、JMJD2B的基因和蛋白表达水平成显著正相关;ERα、ERβ分别与JMJD2A、JMJD2B的基因及蛋白表达水平成显著正相关。结论:1、绝经后骨质疏松组骨组织中组蛋白去甲基化酶JMJD2A、JMJD2B表达水平明显低于骨量正常组,骨密度与JMJD2A、JMJD2B表达水平成显著正相关,提示组蛋白去甲基化酶JMJD2A、JMJD2B可能参与骨代谢过程,能够降低绝经后骨质疏松症的发生。2、两组患者骨组织中ER α、ER β表达水平与组蛋白去甲基化酶JMJD2A、JMJD2B存在显著正相关,提示JMJD2A和JMJD2B可能通过调控雌激素-ER信号通路调节骨代谢,它们之间的具体交通机制有待阐明。3、组蛋白去甲基化酶JMJD2A、JMJD2B可能作为肾虚型绝经后骨质疏松症的一种括抗酶。
[Abstract]:Objective: the expression levels of histone bone in osteoporosis to gene and protein methylation enzymes JMJD2A, JMJD2B and estrogen receptor alpha.ER beta ER observation of kidney deficiency after menopause, and analysis of histone demethylase JMJD2A, JMJD2B and bone mineral density and the correlation between JMJD2A, JMJD2B and ER alpha, ER beta in order to find the correlation, histone methyltransferase JMJD2 in postmenopausal osteoporosis with kidney deficiency after contact, as a new target for the treatment of postmenopausal osteoporosis. Methods: in the Department of orthopedics of the First Affiliated Hospital of Guangzhou University of Chinese Medicine from October 2014 to June 2015 30 cases of postmenopausal female patients with hip osteoarthritis undergoing total hip replacement, aged 50 to 70 years of age, preoperative use of dual energy X-ray absorptiometry measurement in patients with femoral neck BMD, divided into control group of 15 cases (i.e. normal bone mass group, femoral neck BMD T value -1.0) and 15 cases in the experimental group (i.e. postmenopausal osteoporosis group, femoral neck BMD T value is less than or equal to -2.5), extraction operation side at the femoral neck cancellous bone reserve. The expression of JMJD2A, JMJD2B, ER -, and ER beta gene and protein in cancellous bone tissue was detected by real time fluorescence quantitative PCR and Western blot. The experimental data were treated with SPSS20.0 software, and P0.05 was found to be statistically significant. Pearson correlation analysis was used to analyze the correlation between JMJD2A and JMJD2B gene and protein expression and bone mineral density, and to analyze the correlation between JMJD2A, JMJD2B and ER alpha, ER beta gene and protein expression. Results: 1. Compared with normal bone mass group, the expression levels of JMJD2A, JMJD2B, ER and ER ER in bone tissue of postmenopausal osteoporosis group were significantly decreased, and the difference between groups was statistically significant (P = 0.01). Compared with the normal bone mass group, the expression level of OPG mRNA in the bone tissue of postmenopausal osteoporosis group decreased, and the expression level of RANKL and RANKL/OPG mRNA increased. The difference between the two groups was statistically significant (P = 0.01). 2, correlation analysis showed that BMD of femur neck was positively correlated with JMJD2A and JMJD2B gene and protein expression level in postmenopausal women, and ER and ER were significantly positively correlated with JMJD2A and JMJD2B gene and protein expression level. Conclusion: the osteoporosis bone tissue, 1 group of histone demethylase JMJD2A after menopause, the expression level of JMJD2B was significantly lower than that of normal bone mass group, the expression of bone mineral density and JMJD2A, JMJD2B level was positively correlated, suggesting that histone demethylase JMJD2A and JMJD2B may be involved in bone metabolism process, can reduce the incidence of osteoporosis postmenopausal. 2, two groups of patients in bone tissue ER alpha, ER beta expression level and histone demethylation JMJD2A and JMJD2B have a significant positive correlation, suggesting that JMJD2A and JMJD2B may regulate the bone metabolism through estrogen -ER signaling pathway, and the specific traffic mechanism between them needs to be elucidated. 3. Histone demethylation enzyme JMJD2A and JMJD2B may be an anti enzyme of kidney deficiency postmenopausal osteoporosis.
