胸痹痰阻心脉证和阴寒凝滞证与CYP2C19基因代谢型分布相关性研究

【摘要】:目的:研究胸痹痰阻心脉证及阴寒凝滞证患者CYP2C19基因代谢型分布特点,并探索两组中医证型与基因代谢型之间的相关性。方法:通过对2015年5月-2016年1月期间,江苏省中医院心内科住院病人进行中医辨证分型,共收集符合中西医诊断标准的患者169例。住院期间测定其CYP2C19基因型及相关实验室指标,并将患者的检查结果、病史资料、基本信息等记录在临床资料信息收集表中。待研究结束时,对相关数据进行整理归纳,并用SPASS软件处理,分析出胸痹痰阻心脉证及阴寒凝滞证患者CYP2C19基因代谢型分布特点,以及两组中医证型与基因代谢型之间的相关性。结果:169例病人中CYP2C19基因代谢型分布:快代谢68例(40.2%)、中等代谢79例(46.8%)、慢代谢22例(13%)。快代谢在痰阻心脉证中所占比例为44.1%,在阴寒凝滞证中所占比例为19.2%,两者之间的分布差异具有统计学价值(P=0.0180.05),更倾向于在痰阻心脉证中表达;中等代谢型在痰阻心脉证中所占比例为42.7%,在阴寒凝滞证中所占比例为69.2%,两者之间的分布差异具有统计学价值(P=0.0120.05),更倾向于在阴寒凝滞证中表达。结论:CYP2C19基因代谢型以快代谢和中等代谢为主;痰阻心脉证与阴寒凝滞证及CYP2C19基因代谢型存在相关性,快代谢更倾向于在痰阻心脉证中表达,中等代谢更倾向于在阴寒凝滞证中表达,其内在机制尚需进一步研究。
[Abstract]:Objective: to study the distribution characteristics of CYP2C19 gene metabolic type in patients with syndrome of phlegm obstruction of heart pulse and syndrome of Yin cold stagnation. And to explore the two groups of TCM syndromes and gene metabolism type correlation. Methods: from May 2015 to January 2016, Jiangsu Provincial Hospital of traditional Chinese Medicine, Department of Cardiology inpatients were classified by TCM syndrome differentiation. A total of 169 patients who met the diagnostic criteria of traditional Chinese and western medicine were collected and their CYP2C19 genotypes and related laboratory indexes were determined during hospitalization. The examination results and medical history of the patients were also analyzed. The basic information was recorded in the clinical information collection table. At the end of the study, the related data were summarized and processed with SPASS software. To analyze the distribution characteristics of CYP2C19 gene metabolism in patients with syndrome of phlegm obstruction of heart pulse and syndrome of Yin cold stagnation. Results the distribution of CYP2C19 gene metabolic type in 169 cases: fast metabolism 68 cases 40.2%). There were 79 cases of moderate metabolism and 22 cases of slow metabolism. The proportion of fast metabolism in phlegm blocking heart pulse syndrome was 44.1, and that in Yin cold stagnation syndrome was 19.2%. The difference of distribution between the two groups has statistical value. It is more likely to be expressed in phlegm blocking heart and pulse syndrome. The proportion of moderate metabolic type in phlegm blocking heart vein syndrome was 42.7 and that in Yin cold stagnation syndrome was 69.2%. The distribution difference between the two has statistical value (P0.0120.05). Conclusion the metabolic type of CYP2C19 gene is mainly fast metabolism and moderate metabolism. There was a correlation between phlegm blocking heart pulse syndrome and Yin cold stagnation syndrome and metabolism type of CYP2C19 gene. Fast metabolism was more inclined to express in phlegm blocking heart vein syndrome and moderate metabolism was more inclined to express in Yin cold stagnation syndrome. Its internal mechanism needs further study.
