针刺结合五苓汤加减治疗强直性脊柱炎的临床研究

【摘要】：目的:观察针刺结合五苓汤加减治疗强直性脊柱炎的疗效及安全性。方法:将108例强直性脊柱炎患者按随机数字表法分为西药组、中药组和针药组各36例。西药组给予柳氮磺吡啶片,中药组给予五苓汤加减,针药组给予针刺结合五苓汤加减,疗程均为60 d,观察各组患者治疗前后总和指数(BAS-G)、活动性指数(BASDAI)、放射学指数(BASRI)、活动度衡量指数(BASMI)和中医辨证强直性脊柱炎(中医证候)的评分,检测治疗前后血沉(ESR)、C-反应蛋白(CRP)、甲状旁腺激素(PHI)和补体3(C_3)的变化,比较各组临床疗效及不良反应发生率。结果:治疗后针药组总有效率71.4%,优于西药组的58.1%和中药组的52.8%,针药组各评分均较西药组和中药组改善更为显著,针药组和中药组不良反应发生率低于西药组。结论:针刺结合五苓汤加减可显著提高强直性脊柱炎患者BAS-G、BASDAI、BASRI、BASMI和中医证候评分,改善ESR、CRP、PHI和C_3水平且安全有效。
[Abstract]:Objective: to observe the efficacy and safety of acupuncture combined with Wuling decoction in the treatment of ankylosing spondylitis. Methods: 108 patients with ankylosing spondylitis were randomly divided into western medicine group. There were 36 cases in the traditional Chinese medicine group and 36 cases in the acupuncture and medicine group. The western medicine group was given sulfasalopyridine tablet, the traditional Chinese medicine group was treated with Wuling decoction, and the acupuncture and medicine group was treated with acupuncture combined with Wuling decoction for 60 days. The index of summation before and after treatment, the index of activity and the index of radiology before and after treatment were observed. The activity index of basic) and the scores of TCM syndrome differentiation ankylosing spondylitis (TCM syndrome) were measured before and after treatment with ESR C- reactive protein (CRP). The changes of parathyroid hormone (PHIH) and complement 3 (C3) were compared to compare the clinical efficacy and incidence of adverse reactions. Results: the total effective rate of acupuncture and medicine group was 71.4% after treatment. It was better than the western medicine group (58.1%) and the traditional Chinese medicine group (52.8%). The scores of the acupuncture and medicine group were better than that of the western medicine group and the traditional Chinese medicine group. Conclusion: acupuncture combined with Wuling decoction can significantly improve basic basic basic RI in patients with ankylosing spondylitis. BASMI and TCM syndromes score improved the level of BASMI CRP PHI and C _ (3), and were safe and effective.
