加味泻黄散治疗小儿疱疹性咽峡炎（心脾积热型）的临床研究

【摘要】：目的:通过运用加味泻黄散治疗小儿疱疹性咽峡炎,观察其疗效,评估此方在治疗心脾积热型小儿疱疹性咽峡炎中的应用价值,并对其疗效优势做出总结,为今后制定合理的治疗方案,进一步探讨如何治疗小儿疱疹性咽峡炎及发扬中医药在疱疹性咽峡炎中的治疗优势提供临床及理论依据。方法:将80例符合要求的患儿采用简单随机化方法分为两组,分别为试验组、对照组,每组各40例。试验组给予加味泻黄散治疗,对照组给予西医常规综合治疗。观察两组患儿治疗前后各项临床症状体征、实验室指标的变化,分别对两组患儿治疗前后的症状体征积分进行评定,观察两组患儿疱疹消退时间及退热时间,并对其疗效做出评价。成果:两组疱疹性咽峡炎患儿的年龄、性别分布、治疗前的病情程度、各项症候积分等方面均无明显差异(P0.05),具有可比性。治疗后试验组在改善患儿疱疹消退时间及退热时间方面均优于对照组,其差异具有统计学意义(P0.05);试验组在改善患儿发热、疱疹数量、头身疼痛、咳嗽、饮食方面均优于对照组,差异具有统计学意义(P0.05);而在改善咽痛、吞咽状况、面色、精神、大便方面,差异无统计学意义(P0.05)。两组病例治疗前后各项积分差值比较,试验组在改善患儿咽痛、精神、饮食方面优于对照组(P0.05),具有统计学意义,其余方面差异无统计学意义(P0.05)。治疗后总积分的组间、组内比较均具有统计学意义(P0.05),且试验组总的症状体征改善状况优于对照组。两组患儿进行疾病疗效分析时,试验组临床痊愈6例(15%),显效19例(47.5%),有效14例(35%),总体疗效明显优于对照组(P0.05)。结论:运用加味泻黄散治疗心脾积热型小儿疱疹性咽峡炎较西医常规综合治疗方法能明显提高临床疗效,可有效缩短疱疹消退时间及退热时间,改善多项症状及体征评分。
[Abstract]:Objective: to observe the curative effect of Jiawei Xiehuang Powder in treating herpes isthmus in children, to evaluate the value of this prescription in the treatment of infantile herpes spondylitis of heart and spleen and heat type, and to summarize its curative effect advantage. To establish a reasonable treatment plan for the future. To further explore the treatment of herpes isthmus in children and to carry forward the advantages of traditional Chinese medicine in the treatment of herpes isthmus to provide clinical and theoretical basis. Eighty children who met the requirements were randomly divided into two groups by simple randomization method. Experimental group, control group, each group of 40 cases. The experimental group was treated with Xiehuang powder, the control group was given routine western medicine treatment. The two groups of children before and after treatment of clinical symptoms and signs. The changes of laboratory indexes were evaluated before and after treatment, and the time of herpes regression and fever were observed. Results: there was no significant difference in age, sex distribution, degree of illness before treatment, symptom score and so on between the two groups (P 0.05). After treatment, the treatment group was superior to the control group in improving the time of herpes regression and fever, and the difference was statistically significant (P 0.05). The treatment group was superior to the control group in improving fever, herpes quantity, head pain, cough and diet, and the difference was statistically significant (P 0.05). But in improving pharynx, swallowing condition, face, spirit, defecation, there is no significant difference between the two groups before and after treatment, the difference between the two groups before and after treatment, the test group in improving the pharynx pain, spirit. Diet was better than the control group (P 0.05), with statistical significance, the other aspects of the difference was not statistically significant (P 0.05). After treatment, the total score between groups. In the analysis of the curative effect of the two groups, there were 6 cases of clinical recovery in the experimental group (15 cases). The remarkable effect was 47.5% in 19 cases and 35% in 14 cases. Conclusion: the treatment of infantile herpetic isthmus caused by heart and spleen accumulation fever in children with modified Xiehuang Powder can obviously improve the clinical effect compared with the routine comprehensive treatment of western medicine. It can effectively shorten the time of herpes extinction and fever, and improve the scores of multiple symptoms and signs.
