基于“脾虚毒损胃络”假说应用健脾消毒饮治疗慢性萎缩性胃炎癌前病变的临床研究

【摘要】：目的:观察健脾消毒饮治疗慢性萎缩性胃炎癌前病变的临床疗效,并从理论和临床两方面探讨其作用机制,为中医药从“毒”论治CAG及胃癌前病变提供新的思路与方法。方法:选择符合纳入标准的CAG癌前病变患者60例,随机分为2组,即试验组和对照组,各30例。试验组给予健脾消毒饮口服治疗,日一剂;对照组给予胃复春片口服治疗,一次4片,一日3次。两组均以1月为1疗程,共3疗程。观察治疗前后两组患者的临床症状、胃镜象、病理组织学、幽门螺杆菌、血流变、表皮生长因子(EGF)等变化情况。结果:治疗结束后,两组病例临床总有效率比较,试验组为90.00%,对照组为70.00%,差异有统计学意义(P0.05);试验组萎缩、肠化及异型增生等病理组织学积分改善情况明显优于对照组(P0.05);试验组与对照组的Hp根除率分别为55.56%、47.37%;两组EGF表达情况较前改善(P0.05);两组血流变情况均较治疗前明显改善(P0.05),且试验组疗效更优(P0.01)。结论:基于“脾虚毒损胃络”的健脾消毒饮是治疗慢性萎缩性胃炎癌前病变的有效方药。在改善临床症状,阻断、甚或逆转CAG及胃癌前病变之发生发展,具有重要指导意义。
[Abstract]:Objective: to observe the clinical effect of Jianpi xiaoyin in treating precancerous lesions of chronic atrophic gastritis, and to explore its mechanism from both theoretical and clinical aspects. Methods: 60 patients with precancerous lesions of CAG were selected and randomly divided into two groups: experimental group and control group. 30 cases in each group were given oral treatment with Jianpi disinfectant drink once a day. The control group was treated with Weifuchun tablet orally, 4 tablets once, 3 times on 1st. The two groups were treated with January as a course of treatment for 3 courses. The clinical symptoms, gastroscopy and histopathology of the two groups were observed before and after treatment. Results: after the treatment, the total effective rate of the two groups was 90.00%. The control group was 70.00g, the difference was statistically significant (P 0.05). The improvement of histopathological scores such as atrophy, intestinal metaplasia and dysplasia in the experimental group was significantly better than that in the control group (P 0.05). The eradication rate of HP in the test group and the control group was 55.56 and 47.37 respectively. The expression of EGF in the two groups was better than that in the former group. The hemorheology in both groups was significantly improved than that before treatment (P0.05). Conclusion: Jianpi disinfection decoction based on "spleen deficiency and toxin damage stomach collaterals" is an effective prescription for the treatment of precancerous lesions of chronic atrophic gastritis. Even reversing the occurrence and development of CAG and gastric precancerous lesions has important guiding significance.
