曾敬光教授学术思想及临床经验总结（附：124例妊娠恶阻临床回顾性分析）

【摘要】：[研究背景]川派中医长期以来在中医药界占有举足轻重的地位,为更好地传承中医药文化发展,2012年四川省中医药管理局组织和开展了川派中医药名家学术思想及临床经验研究项目。曾敬光(1918-2010),女,四川省双流县人,成都中医药大学教授。长年耕耘在教学、临床第一线,多次承担教学和教材编写工作。其在中医妇科方面具有独到见解。[研究目的]1整理、总结曾敬光教授的学术思想及临床经验,提炼其临床辨证、辨病特点及拟方潜药规律。2回顾性分析124例妊娠恶阻入院患者临床资料,探索妊娠恶阻中医证型分布及临床用药规律。[研究方法]1通过图书馆、互联网、数据库、名医及曾敬光教授家眷典藏等途径挖掘有关曾敬光教授的论文、著作、医案等资料,归总曾敬光教授的学术思想、临证经验。2通过回顾性的临床研究,搜集2012年6月到2016年01月自电子病历系统使用以来成都中医大学附属医院妇科124例妊娠恶阻患者原始资料,将上述资料填入“妊娠恶阻临床调查表”中,并输入EXCEL软件进行原始数据分类整理,运用统计软件包SPSS17.0,进行计数资料分析、聚类分析、因子分析及相关性分析。[研究结果及结论]1学术思想研究结果及结论(1)强调“冲任损伤是妇科疾病的基本病机和最终病位”,从冲任不固、不盛、失调、阻滞四证统率月经不调诸症。(2)主张“既要治病,又要安胎”的妊娠病治疗原则,提出恶阻转归三期,强调从肾气、冲任、气血论治胎漏、胎动不安,从脾虚肝郁论治妊娠肿胀等。(3)擅“五证为纲”论治不孕症。2妊娠恶阻临床回顾性分析研究结论(1)妊娠恶阻为妇女妊娠期多发病、常见病,临床病情严重程度以中度多见,中医临床分型以脾胃虚弱型较多,其次为肝胃不和型。(2)本研究治疗妊娠恶阻在顾护脾胃的基础上强调脾胃、肺、肝同治,兼以补肾。用药以补益药为主,止咳化痰平喘药、理气药次之,甘温并用,苦辛并用,寒热同调,结果表明中西医结合能明显改善恶心、呕吐、厌食,疲倦乏力等一系列临床证候,具有较好疗效,即中西医结合治疗该病症有一定的临床指导意义。(3)本研究的设计启发于对曾敬光教授关于妊娠恶阻学术思想及临床经验的研究,该研究结果与曾敬光教授治疗经验存在一定程度的一致性,推论曾敬光教授治疗该病的临床经验有很强的实用价值。(4)本课题仅对本次研究结果分析,研究中患者存在复发可能,未行随访,在日后的研究工作中可加强跟踪研究。

Clinical experience .2 through retrospective clinical studies. From June 2012 to January 2016, we collected the original data of 124 pregnant women with adverse pregnancy since the electronic medical record system was used in the department of gynecology of affiliated hospital of Chengdu University of traditional Chinese Medicine. The main medication is tonifying medicine, cough, phlegm and asthma medicine, Qi-regulating medicine, sweet and warm combined, bitter combined, cold and heat homology, the results show that integrated Chinese and Western medicine can significantly improve nausea, vomiting, anorexia. A series of clinical symptoms such as fatigue and fatigue have good curative effect. The design of this study is inspired by Professor Zeng Jingguang's academic thought and clinical experience on pregnancy obstruction. The results of this study are consistent with Professor Zeng Jingguang's treatment experience to a certain extent. It is concluded that Professor Zeng Jingguang's clinical experience in the treatment of the disease is of great practical value. Patients in the study had the possibility of recurrence and were not followed up, and the follow-up study could be strengthened in the future research work.
